In keeping with the Hospice/Home Healthcare philosophy, the Physical Therapist performs all phases of physical therapy evaluation, treatment planning, treatment and documentation, and performs other tasks related to quality of care and effective department function. Duties include evaluating patients, participating in treatment program planning; administering physical therapy modalities, procedures and tests; documenting patient records and progress notes; maintaining appropriate contact with the referring physician and co-workers. Ensures a clean and safe work environment by identifying unsafe conditions and malfunctioning equipment and correcting them; supervises Certified Home Health Aides in the participation in the overall treatment program. Assists DPCS, Hospice Supervisor, and QI Coordinator with QI as required. Performs other related duties, including in-service education, clerical and other tasks relevant to effective department function as necessary.
Performs patient initial and reevaluations; establishes and implements treatment plan accordingly.
Administers treatment plan using accepted therapy procedures; oversees therapist extenders (aides) in the implementation of established treatment plans.
Teaches patient and/or caregiver in functional activities and treatment techniques of physical therapy as appropriate to meet their needs.
Job Details
Patient Care: Performs patient initial and reevaluations; establishes and implements treatment plan accordingly. Administers treatment plan using accepted therapy procedures; oversees therapist extenders (aides) in the implementation of established treatment plans. Teaches patient and/or caregiver in functional activities and treatment techniques of physical therapy as appropriate to meet their needs.
Documentation: Documents daily on the appropriate forms using clear and concise data. Records treatments accurately and communicates patient status/progress in appropriate forums. Submits documentation regularly.
Communication Skills: Communicates regularly with other members of the medical team regarding goals, progress, and rehabilitative potential. Works as a collaborative member of the treatment team.
Standards of Conduct: Maintains the standards of conduct as promulgated by the American Physical Therapy Association; adheres to the professional code of ethics in the performance of duties.
Clinical Knowledge: Demonstrates specialized clinical knowledge and skills in an identified area of expertise.
Compliance with policy and procedures: Complies with safety, infection control, Dignity Health Integrity Program, and other organizational policies. Completes department orientation and annual competency checks; adheres to dress code and attendance policies.
Other Staff Responsibilities: Actively supports the organization's mission, philosophy and strategic plan; demonstrates good time management and priority setting.
Job Requirements Experience Required Two years of professional experience in rehabilitation.
Licensure Required Licensed, or eligible for licensure, by the State of California Board of Medical Quality Assurance. Basic Life Support (BLS) certification through the American Heart Association. California driver's license and automobile liability insurance. Contractors must provide proof of professional liability, annual PPD and PE.
Special Skills Required Ability to communicate effectively, both orally and in writing; strong interpersonal skills. Demonstrates knowledge of principles, techniques, and procedures within the scope of practice as a Physical Therapist. Ability to organize and prioritize work efficiently and to function in a team environment with patients, families, and other professionals. Ability to teach physical therapy techniques to patients and caregivers. Able to work with minimal supervision.
