The Patient Navigator builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. The Patient Navigator will work to identify individuals who are HIV+, HepC+, or vulnerable to HIV but are not engaged in medical care. The Patient Navigator will work to reengage patients living with HIV+ and/or HepC+ and those patients vulnerable to HIV and HepC in medical services. Main Duties & Responsibilities Provide accurate information to clients about living with HIV/HepC or taking PrEP, including how to prevent transmission, the benefits and challenges of care and treatment, and available services and how to access them. Accompany patients living with HIV and/or HepC and those patients vulnerable to HIV and HepC to medical or social service appointments as needed. Provide informal counseling, support and follow-up to clients. Assist with patient navigation, benefits enrollment, and health education. Link vulnerable populations to health care/social service resources. Assist clients with enrollment or re-enrollment in medical care, medical benefits programs and other benefits for which they are eligible. Facilitate communication and client empowerment in interactions with health care/social service systems. Plan and facilitate patient educational groups. Qualifications Education High School diploma or equivalent required A minimum two years of work experience in health, social or community services preferred. Skills And Knowledge Bilingual in fluency (English/Spanish) strongly preferred. Local travel is required for the position. CPR certification required. Verbal and written communication skills, including high level of comfort presenting to groups. Microsoft Office computer skills desired.
