Position: RN Case Manager (Travel) Assignment: 13 weeks, Tallahassee, FL Responsibilities
Assess patients' medical, psychosocial, and functional needs Collaborate with interdisciplinary teams to develop patient-centered care plans Develop and update individualized care plans aligned with patient goals and clinical guidelines Coordinate transitions of care across the continuum Facilitate communication among patients, families, and the care team Monitor and evaluate patient outcomes; adjust care plans as needed Ensure compliance with regulatory standards and insurance requirements Advocate for patients and promote equitable access to services Conduct utilization reviews and manage length of stay per evidence-based guidelines Educate patients and families on conditions and available resources Document case management activities in the medical record Participate in interdisciplinary team meetings Adhere to safety, infection control, and regulatory standards Perform other duties as assigned within scope Qualifications
Required One year of recent experience as an RN Case Manager Valid RN license in compliance with state regulations Current BLS (AHA/ARC) certification Preferred ACLS certifications (AHA/ARC) Other certifications and licenses may be required for this position Essential Skills
Critical thinking, service excellence, and strong interpersonal communication English language proficiency in reading, writing, and speaking Ability to read and follow written and verbal instructions; proficient PC skills Physical ability to perform required movements and assist with equipment Effective collaboration with diverse teams and professionalism Ability to handle busy environments and maintain attention to detail Visual acuity and effective communication skills Benefits
