Job Description Summary

Hiring range from $115,000 to $170,000 based on experience. The Clinical Pharmacist is a licensed professional who provides safe and appropriate pharmacy services to Option Care patients in accordance with policies, procedures, and regulatory standards. This is an entry level position into the practice of clinical pharmacy in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff, and other professionals regarding appropriate medication use. Job Responsibilities

Determines the suitability of individual patients for home care. Collects and organizes patient-specific information to determine suitability for home care according to Option Care policies. Assesses patient suitability, medical condition, prescribed therapy, and prognosis with defined outcome goals. Designs, recommends, monitors and evaluates patient-specific pharmacy care plans for pharmacotherapy in home care. Makes recommendations for therapy or monitoring parameter modifications as appropriate; collects and organizes information to prevent, detect, and resolve medication-related problems; identifies monitoring parameters; completes nutritional assessment and provides recommendations. Modifies care plans based on monitoring data and other pertinent information. Applies pharmacokinetic principles and assesses patient laboratory values. Provides medication-use education to Option Care staff, other professionals, home care patients, and caregivers. Uses effective education techniques for counseling. Ensures continuity of pharmaceutical care between home and other patient-care settings. Communicates pertinent information to/from home and other settings. Prepares and dispenses medications per policy, calibrates equipment, ensures appropriate concentration and storage, and maintains safety and quality assurance practices. Prepares labels and ensures authorization to dispense; complies with applicable laws and DEA regulations; maintains perpetual inventory accuracy for scheduled medications and other drugs subject to diversion. Manages use, maintenance, and troubleshooting of medication administration equipment and related devices. Programs and advises on equipment use; participates in incident management and reports discrepancies in inventory systems. Responds to drug information requests from healthcare providers and patients; bases responses on literature and pursues relevant continuing education to maintain competency and compliance with state requirements. Manages investigational drug products per regulations and Option Care policies; optimizes workflow with pharmacy support personnel and ensures accuracy of their work. Complies with accreditation, legal, regulatory, and safety requirements; maintains confidentiality of patient and proprietary information according to policy. Education and Experience

Graduate of an accredited School of Pharmacy. Current, active Pharmacy license as granted by the State Board of Pharmacy. One of the following: at least one year of sterile IV admixture experience; knowledge of non-sterile compounding, immunizations and MTM; completion of an ASHP-accredited Pharmacy Residency Program; completion of an Advanced Pharmacy Practice Experience; or an equivalent experience as approved by leadership. Qualifications

Experience providing customer service to internal and external customers and meeting quality standards. Basic skills in Microsoft Word, Excel, and PowerPoint. Travel Requirements

Willing to travel up to 5% of the time for business purposes (within state). Preferred Qualifications

Completion of an ASHP-accredited Pharmacy Residency Program or an APPE. Experience in home care, hospital, or clean room settings. Management Requirements

At least 1 year of indirect management experience, including assisting in development, training, and assignment of work/projects to other team members. May perform other duties as assigned. Compensation and Benefits

Salary range is $102,221.94 to $170,369.91; pay is determined by education, experience, knowledge, skills, and abilities.
