Program Summary: The Youth ACT team (Assertive Community Treatment) provides mental health services to youth and families at risk of entering or returning to higher levels of care, such as inpatient hospitalization or residential settings, aiming for safety and stabilization. The team collaborates closely to address youth mental health concerns and their impact on the family as a unit. Each member offers individualized treatment through assessment, counseling, and clinical services in various settings including in-home and community-based environments.
Position Summary We seek a compassionate and experienced Psychiatric Nurse Practitioner to join our Youth Assertive Community Treatment (ACT) team. This role is crucial within a multidisciplinary team. The ideal candidate is passionate about community and family work. Responsibilities include psychiatric evaluations, medication management, treatment planning, collaboration with therapists and community organizations, emergency assessments, and participating in case reviews and quality initiatives.
Major Responsibilities/Activities
Provide psychiatric evaluation and medication management to youth and adults in outpatient services.
Review and sign client treatment plans as needed.
Consult with agency staff regarding client treatment; participate in supervision and case reviews.
Complete progress notes following current procedural terminology standards.
Provide medication information for dispensing at home and school when needed.
Communicate medication, behavioral, or health concerns to parents and therapists.
Document concerns and monitor progress monthly.
Participate in program committees such as Incident Review, Case Conferences, and Advisory Boards.
Review laboratory results and discuss concerns with the nurse if necessary.
Collaborate with mental health therapists on behavioral health concerns and outcomes.
Maintain effective communication skills, both written and verbal.
Show respect and courtesy in all interactions.
Support the agency’s mission, values, and philosophy.
Apply problem-solving and conflict resolution skills.
Support and communicate cooperatively with supervisors and coworkers.
Incorporate the agency’s values in decisions and actions.
Demonstrate initiative and serve as a role model.
Maintain and submit required documentation and comply with safety policies.
Promote diversity, fairness, and acceptance in all interactions.
Participate in quality improvement processes.
Minimum Requirements
Recognized Nurse Practitioner credential with current NYS registration.
Board Certified in Psychiatry.
At least two years’ experience with children and families; experience as a Psychiatric Nurse Practitioner preferred.
Ability to relate well to children and staff.
Valid NYS driver’s license and reliable transportation.
Compensation Competitive pay range of $150,000-$175,000 annually, based on experience.
