Position Overview
We are seeking skilled and compassionate
Nurse Practitioners
to conduct home visits in
Manhattan
and surrounding areas . This role offers the opportunity to make a meaningful impact on patient care while enjoying a flexible work environment.
Key Responsibilities
Conduct comprehensive assessments and evaluations of patients in their homes. Develop and implement personalized care plans based on patient needs. Manage and stabilize chronic and acute medical conditions. Provide high-quality medical care and support to patients and their families. Collaborate with other healthcare professionals to ensure optimal patient outcomes. Maintain accurate and timely documentation of patient interactions.
Qualifications
Valid Nurse Practitioner license in New York State. Board certification is required. Must be enrolled as a Medicaid provider. Previous experience in home healthcare or similar settings preferred. Strong clinical assessment and decision-making skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively.
Compensation
Rate: $100 - $150 per visit, depending on visit type
