RN Admissions Nurse
in the Reliant at Home Plano coverage area. This opportunity has an expectation of 15 Admissions/Week, Monday through Friday, and includes a competitive base salary, sign-on bonus, and over-productivity bonus. Must have HCHB experience and strong documentation skills. Reliant at Home is a multi-site, Texas-only Home Health, Hospice, Caregivers, and Rehab company with 14 locations in Texas. Primary function: perform Home Health admissions in the patient home setting. Perform Home Health evaluation and assessment per OASIS E data set. Develop and document the plan of care in correlation with the assessment. Provide skilled nursing care as ordered by physician. Timely submission of all required documentation. Communicate with supervisor, interdisciplinary team, patients/families, and physician in the coordination of care process. Demonstrate commitment, professional growth, and competency. Meet productivity standards. Qualifications
Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing. Current State license as a Registered Nurse, current State Driver's License. Two years of experience as a Registered Nurse in a clinical care setting and 2-3 years of Full-Time Home Health experience. Reliable transportation. Valid and current auto liability insurance.
