Position Summary
The Family Nurse Practitioner, as a licensed Advanced Practice clinician, provides medical care to a panel of RFHN clinic patients under the supervision of a physician following established standards and practices. Advanced Practice Providers are expected to demonstrate excellent clinical judgement, empathic communication, critical thinking skills, care team leadership and appropriate role modeling. This position is a member of the practice team supporting PCMH standards and practices.
Duties And Responsibilities
To be performed in accordance with RFHN Policies and Procedures
Credentialing
Maintains provider credentials up to date and submits copies to administration in a timely manner and as per policy, including licenses, immunizations, annual Tb screen and flu shot. Maintains CME requirements and board certification as per license and medical specialty board.
Clinical Care
Examines, diagnoses and treats patients according to degree, clinical privileging, and credentialing. Provides quality medical care according to established standards and RFHN clinical quality protocols. Performs a complete physical exam and records findings. Collaborates with physicians in managing acute and long-term medical needs of patients. Orders appropriate laboratory and diagnostic procedures. Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures as needed. Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention. Administers medications and injections according to established standards of care. Interviews and advises patients regarding health and illness prevention. Recommends community resources to meet patient and family needs. Performs all duties and services using a quality improvement approach (see TQM policy) to continuously improve care quality and efficiency and lower healthcare costs. Documents on the electronic health record according to standardized protocols that specify templates to use and where to document certain elements to be able to generate required reports for PCMH and others. Uses a SOAP format for each patient problem that supports the assessment and plan and uses standard abbreviations. Documents all patient communication, including phone calls and messages in the patient record. Completes patient care documentation and submits billing encounters within one business day. Maintains and reviews patients records, charts and other pertinent information. Posts tests and examination results. Triages patient telephone calls and provides consultation. Manages medical emergencies to stabilize for transport as appropriate. Provides monitoring and continuity of care between physician visits. Participates in peer review and internal quality control audits. Attends required meetings and participates in committees as requested. Participates in professional development activities and maintains professional affiliations. Maintains patient confidentiality. Other duties as assigned and requested.
Additional Clinical Duties
Assists in covering patients for providers who are out by reviewing labs, returning calls, authorizing prescriptions and other items to ensure continuity of care in general. Reviews “Provider of the Day” box and EHR tasks for providers they are covering. Assists nurses in triaging and sees walk-in patients. Completes online compliance courses.
Work Hours and Productivity Standards
The provider schedule and hours are determined at the start of employment and require approval from the medical supervisor and department manager to change schedule to make sure that patient coverage is maintained. Clinic shifts are Day 8am-5pm, or Evening 10am-7pm, and Saturday 8am-1pm. Must be present for the entire assigned shift to make sure patients are seen unless arranges with other providers for coverage. After orientation and ramp up period, every provider is expected to complete 10 patient visits per each 4-hour session, and will be scheduled 11 patients per 4-hour session, or 22 patients in an 8-hour day. Family Medicine providers participate in outpatient calls according to FTE. Works one Saturday shift per month (unless exempt).
General Employee Duties
Adheres to basic employee duties that include starting work shifts on time and being available for patient care throughout a work shift, completing time cards promptly, submitting leave requests in a timely manner, complies with dress code and professional conduct towards staff and patients (see RFHN Employee Handbook). Follows infectious diseases protocols such as masking and using additional protective gear based on patient and staff interactions. Attends all required staff and provider meetings. If
Reads and responds to email, voicemail, and texts promptly. Participates in committees and improvement efforts, as requested. Participates in quality improvement efforts according to established methodology and protocols. Works with care team to help patients meet health maintenance and chronic disease measures and assists in collecting data for the registry. Provides input into development of medical policies and protocols. Supports RFHN advocacy/outreach and organizational events. Implements and enforces clinic policies and procedures. Maintains patient confidentiality according to HIPAA standards at all times especially in the clinical areas. Obtains prior clearance from Medical Director for guests, students, or residents wishing to work, train or visit RFHN. If representing RFHN at an external event or organization, obtains prior clearance from Medical Director to review presentation.
Qualifications
Graduate of an accredited Master’s Level Nurse Practitioner program. Must have RN/NP licenses from the California Board of Registered Nursing and be in good standing. At least one year of experience as a Nurse Practitioner required; two years of experience highly desirable. Current CPR or BLS or PALS or ACLS. (Note may update upon hiring). Must be credentialed per RFHN policies and procedures for both clinical and billing purposes under Medicare/Medicaid and other payors; must keep credentials current. Bilingual English/Spanish proficiency required for Pediatrics providers; highly desirable for all other departments. Bilingual English and Spanish preferred. Experience in a community clinic setting strongly preferred. Computer literate in a Windows-based environment. Up to date with COVID-19 vaccines per current CDC guidelines strongly recommended.
The salary range for this position is $183,040 to $232,174 annually. However, the final base salary will be determined upon a number of individualized factors such as (but not limited to) the scope and responsibilities of the position, job-related knowledge, skills, experience, education and certification levels, and departmental budget. We also consider internal equity with our current employees when making final offers.
