Evaluate and treat patients in their homes in the Azusa, CA area as a contract Physical Therapist (PT). Coordinate with support staff, case managers, and office staff to manage patient needs and ensure plans of care are carried out. Utilize Omni Therapy’s resources to manage administrative tasks, including notes and scheduling, with assistance from care coordinators and case managers when needed. Maintain communication with the patient and care team to support patient goals and continuity of care. Company highlights & benefits
