Overview
The Alaska Native Tribal Health Consortium is a non-profit Tribal health organization designed to meet the unique health needs of Alaska Native and American Indian people living in Alaska. In partnership with the Alaska Tribal Health System, ANTHC provides comprehensive medical services, wellness programs, disease research and prevention, rural provider training, and related services. Summary: In conjunction with providers from multiple specialties and/or sub-specialties, helps to develop a plan of care for a patient with a potential or actual new diagnosis of cancer. Under general direction collects and analyzes data, identifies expected outcomes, develops a plan of care with interventions, and continuously evaluates progress toward the outcomes, working collaboratively with the specialty healthcare team and other disciplines. Anticipated role entails coordinating care to support patients through evaluation and treatment processes across the health system. Responsibilities
Provides state-wide wrap around services for patients who are being evaluated for or diagnosed with cancer. Uses critical thinking and problem solving to coordinate health care services for patients undergoing cancer evaluation or recently diagnosed with cancer. Uses advanced nursing knowledge to assess, plan, implement, evaluate, and interact with patients and their caregivers. Communicates with all members of the healthcare team on behalf of the patient. Translates complex medical information into understandable language to support informed patient and family care choices. Coordinates appointments, travel, and lodging to optimize timely evaluation and start of care. Attends ANMC Tumor Board weekly to obtain a comprehensive treatment plan. Evaluates options and services to meet individual health needs, promoting quality, cost-effective outcomes; may include development and maintenance of patient tracking and reporting programs to identify gaps in care. Independently uses nursing knowledge to collect data, determine diagnoses, set individualized outcomes, and implement a plan of care in collaboration with the healthcare team. Adheres to Standards of Practice for Case Management and the ANA Code of Ethics; contributes to professional development within a Shared Governance nursing practice model. Collaborates with patients, families, and other providers to establish goals and plans of care that preserve autonomy, dignity, and rights. Assesses safety, effectiveness, evidence-based practice, and cost considerations in planning and delivering patient care; assists in coordinating insurance payers and services as appropriate. Plans and coordinates with external facilities for complex subspecialty needs not provided at ANMC; triages patients via telephone, email, and in-person encounters. Delivers patient education on complex issues and disease processes; provides timely nursing care according to established standards. Performs program-specific nursing assessments, interventions, and evaluations, including specialized infusions and procedures. Shares specialty information with colleagues to enhance nursing quality of care. Performs other duties as assigned. Knowledge and Skills
Knowledge of nursing science, principles, techniques, and procedures for patient care. Knowledge of healthcare terminology, anatomy and physiology, and disease concepts. Knowledge and application of evidence-based practice to develop care plans and interventions. Knowledge of the hospital environment and how services interact. Knowledge of Alaskan culture. Knowledge of nursing theory and ability to provide theory-based care. Knowledge of shared governance with shared-decision making. Critical thinking skills to provide effective, compassionate nursing care and to assess patient situations. Skill in cardiopulmonary resuscitation and in delivering effective nursing services. Skill in assessing and prioritizing multiple requests from patients, families, and team members. Skill in evaluating health problems and making evidence-based recommendations. Skill in operating a personal computer and using various software applications. Minimum Education Qualification
Associates degree in Nursing. Equivalent knowledge, experience, education, or training may be substituted for education. Minimum Experience Qualification
Non-supervisory - Five (5) years professional work experience in health education, health-related research, and/or health-related program development, implementation and evaluation. An equivalent combination of relevant education and/or training may be substituted for experience. Minimum Certification Qualification
Current registered nurse license in the State of Alaska. Current Basic Life Support card required. Preferred Education Qualification
Bachelor’s Degree in Nursing. Preferred Experience Qualification
Experience in the Alaska Tribal Health System. Preferred Certification Qualification
National certification in Case Management through the ANCC and/or other specialty-specific certification. Employment type
