The Family Nurse Practitioner (FNP) will play a vital role in delivering primary care services in an outpatient setting. This position is ideal for a clinician passionate about addressing health disparities and making a meaningful difference in the lives of patients who may face barriers to care. The FNP will collaborate with a multidisciplinary team to promote wellness, manage chronic conditions, and provide preventative and acute care services.
Responsibilities
Provide comprehensive primary care services for patients of all ages, including health assessments, diagnosis, treatment, and follow-up.
Manage acute and chronic conditions, emphasizing patient education and self-management strategies.
Order and interpret diagnostic tests, prescribe medications, and develop individualized care plans.
Collaborate with physicians, nurses, behavioral health specialists, social workers, and other team members to ensure coordinated and holistic care.
Promote preventative health measures, including screenings, immunizations, and lifestyle counseling.
Document patient encounters accurately and timely in the electronic health record (EHR).
Serve as an advocate for patients, connecting them with community resources and support services as needed.
Participate in quality improvement initiatives and contribute to advancing health equity within the clinic and community.
Qualifications
Current, unrestricted license as a Family Nurse Practitioner in Maryland
National Board Certification as a Family Nurse Practitioner (FNP-BC or FNP-C)
Master’s or Doctorate degree in Nursing from an accredited program
Minimum of 2 years experience in an outpatient, primary care, or community health setting preferred
Strong commitment to serving diverse, underserved, and underprivileged populations
Excellent communication, collaboration, and patient engagement skills
Bilingual or multilingual skills a plus
