Position Overview: The Peer Case Manager is a role within Alma y Esperanza within the Certified Community Behavioral Health Center (CCBHC) at La Familia. This program provides a whole-person, integrated care model that combines behavioral health and social service navigation with coordination of primary health services, aiming to improve outcomes in Alameda County. The Peer Case Manager will provide direct services to participants, offering either solution-focused, short-term support or longer-term case management support, based on an assessment and service plan. The role also supports the design and implementation of outreach and community engagement activities.

Shift : Monday–Friday, 9:00 a.m.–5:00 p.m. with an 11:00 a.m.–7:00 p.m. shift required 1–2 weekdays per week. This role upholds the agency’s mission, vision, and values and contributes to a collaborative and inclusive work culture.

Responsibilities

Case Management and Individualized Care Coordination: Manage a caseload of clients to provide and track case management and care coordination services, including developing and maintaining rapport with participants and using Motivational Interviewing techniques.

Provide trauma-informed, culturally responsive responses to participants and partners; conduct initial assessments and collaborate with the clinical team to finalize assessments.

Help participants identify and stay motivated toward personal objectives within the case plan; facilitate planning, oversee implementation, and monitor case plan objectives.

Provide outreach, engagement, linkage, and follow-up services to clients; act as a role model to inspire, engage, and advocate for clients’ wellness and rights.

Research and provide referral resources; support transitions of care, including hospital discharge and changes in care levels with timely information and options for participants.

Provide crisis intervention as needed, including de-escalation and stabilization, escalating to clinical consultants as appropriate.

Short Term Care Coordination and Follow-Up: Collaborate with the care team to help participants create plans and overcome barriers to access care; assist with appointment scheduling and reminder/follow-up calls to verify participation.

Community Engagement and Outreach: Design, implement, and track activities at the CCBHC site and in the community to improve access to clinical services, validate stressors, support non-clinical wellness, educate on mental health resources, and engage as community stakeholders.

Maintain networking and referral relationships with social service partners and other providers; provide timely outreach in response to referrals from funders and partners.

Other duties: Complete documentation and progress notes in Salesforce; maintain records per funder and program requirements; attend meetings and training; track key metrics; participate in special projects as appropriate.

Qualifications & Experience

Bachelor’s Degree in Community Development, Sociology, or related field with 2+ years of direct service case management or social service experience (education may be waived with 4+ years of experience).

Availability to work 11:00 a.m. – 7:00 p.m. 1–2 days per week.

Bilingual Spanish/English strongly preferred.

Certification: Mental Health Services Administration Medi-Cal Peer Support Specialist preferred or willing to participate in certification training within 6 months of hire.

Experience with low-income and vulnerable communities; ability to work independently with strong critical thinking and professional judgment.

Excellent public speaking and writing abilities; nonprofit experience desirable.

Customer service orientation; ability to interact with people at all levels; discretion with confidential information.

Proficiency with MS Office and Google Drive; valid CA driver’s license; auto insurance may allow business use of a personal vehicle.
