Responsibilities
Provide direct patient care and coordinate primary care for residents in skilled nursing and senior living facilities, focusing on wellness promotion, chronic disease management, and care coordination with other healthcare providers.
Perform comprehensive assessments, develop treatment plans, and provide ongoing care management.
Conduct routine rounds to assess residents’ conditions, adjust treatment plans, and address emergent health concerns.
Collaborate with facility staff, family members, and other healthcare providers to ensure comprehensive, coordinated care tailored to residents’ needs.
Educate residents, families, and facility staff on disease prevention, wellness, and managing chronic illnesses.
Maintain accurate, timely documentation in the EMR, ensuring regulatory and organizational compliance.
Participate in quality improvement initiatives to enhance care delivery and patient outcomes.

Qualifications
Valid nurse practitioner or physician assistant license in state of operation and current board certification
Master’s Degree in Nursing or Physician Assistant Studies
Strong leadership, interpersonal, and problem-solving skills
Excellent written and oral communication skills
Punctuality, organizational skills, and ability to function within a team
This role requires travel to and from one or more assigned facilities. Depending on travel amount, Curana may require a valid driver’s license, current auto insurance, and an acceptable driving record. If so, you will be given 30 days notice. Periodic motor vehicle (MVR) checks may be conducted as a condition of continued employment.
