Overview
CenterWell Home Health is hiring a RN Case Manager, Home Health. You will provide personalized, one-on-one care that helps patients regain independence in their homes. You will work closely with a team of physicians and clinicians to develop and manage care plans that support recovery and help patients return to the life they love. Responsibilities
Provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer ongoing care and case management for each patient, with follow-up as directed by the Clinical Manager. Confer with the physician to develop the initial plan of treatment based on orders and initial assessment; provide hands-on care, management, and evaluation of the plan; revise the plan with physician input as needed by policy/regulation. Coordinate appropriate care with various healthcare personnel (e.g., Physical Therapists, Occupational Therapists, Home Health Aides, and external providers). Report patient care/condition/progress to the physician and Clinical Manager on a continuous basis. Implement patient care plans with patients and families to achieve optimal outcomes. Coordinate/oversee/supervise Home Health Aides, Certified Home Health Aides and Personal Care Workers; provide written care plans and monitor documentation completion as part of supervisory responsibilities. Discharge patients after consultation with the physician and Clinical Manager; prepare and complete required clinical documentation. Prepare medical documentation on all patients, including case conferences, patient contacts, medication changes, recertifications, progress updates, and care plan changes; prepare visit/shift reports and confer with other health care disciplines to ensure optimum patient care. Qualifications
Diploma, Associate, or Bachelor Degree in Nursing A minimum of one year of nursing experience preferred Strong med-surg, ICU, ER, or acute care experience Home Health experience is a plus Current and unrestricted Registered Nurse licensure Current CPR certification Strong organizational and communication skills A valid driver’s license, auto insurance, and reliable transportation are required Pay and Schedule
Base pay range: $70,500.00/yr - $96,900.00/yr Pay range per visit/unit: $45.00 - $63.00 Full-time schedule: 40 hours per week Benefits and About Us
