We’re seeking a Home Health Patient Services Coordinator to help ensure our care teams are scheduled, supported, and set up for success so our patients receive timely, compassionate, and continuous care. What You’ll Do as a Home Health Patient Services Coordinator
Report directly to the Administrator or Executive Director and oversee the day‑to‑day coordination of care services. Schedule and manage visits across all disciplines and ensure proper alignment with physician orders and patient needs. Communicate with field clinicians, patients, physicians, and caregivers to ensure seamless care coordination and service updates. Manage calendar components, including service codes, clinician assignments, and visit timelines. Track and respond to rescheduled, declined, missed, or reassigned visits, ensuring all patients receive appropriate and timely follow up. Support internal operational performance and ensure visits are being completed and documented efficiently. Play a key role in ensuring regulatory compliance, documentation accuracy, and patient satisfaction. Serve as a central communication point for clinical operations, working closely with both office and field teams. About You
Qualifications – What You’ll Bring
High school diploma or GED. 3–5 years of administrative experience; 1+ year of home health experience required. Working knowledge of home health disciplines and visit coordination. Familiarity with Medicare/Medicaid, managed care guidelines, and accreditation standards (ACHC, CHAP, or Joint Commission). Excellent time management and multitasking skills in a fast‑paced clinical environment. Strong written and verbal communication skills, especially when relaying updates to clinicians or patients. Detail‑oriented, deadline‑driven, and collaborative. Preferred Experience (Not Required)
Associate's or Bachelor's degree. EMR scheduling (HomeCare HomeBase or similar). Clinical coordination or intake/scheduling in home health or hospice. Customer service or call center work in a healthcare setting. Familiarity with HR processes or clinician credentialing. We Offer
