Nurse Practitioner
to join our Hospice/Palliative Care team at the Hospice House in Bloomington, Indiana. IU Health Palliative Care is focused on improving quality of life while dealing with a serious illness and providing personalized care that is focused on the patient, not just their disease, supporting care goals through pain and symptom management, spiritual and psychosocial counseling and assistance in advance care planning. We aim to support the patient and their loved ones.
Responsibilities
PRN hybrid opportunity with limited on-site requirements
Provide clinical consults with patient/family members at the Bloomington, Indiana Hospice House (rounding within 24 hours of admission)
48-hour on-call (at home/on phone) coverage - Saturday 8:00am to Monday 8:00am
Key responsibilities include end of life care, medication management, H&P/clinical notes and symptom control
Team includes one Physician and one Advanced Practice Provider
Qualifications
Must hold either ACNP, AGNP or FNP certifications
APP Experience in Palliative or Hospice Care highly preferred
If new graduate, RN experience in Hospice/Palliative Care is required
Must have valid driver's license and reliable transportation as driving is required
