SUMMARY We are currently seeking a
Pharmacist
to join our Specialty Pharmacy team. This position is part time, 40 hours per pay period, day shift. The pharmacist will be responsible for pharmacy functions such as checking and verifying prescriptions, evaluating for drug interactions, and assessing medication profiles. Advanced functions include accessing and interpreting lab results, documenting outreach calls in the EHR, evaluating adherence issues, and communicating with specialty clinic providers and staff. The pharmacist will connect with patients to provide medication education for various specialty and non‑specialty disease states and link them to care within our Specialty Pharmacy.
RESPONSIBILITIES
Verifies, prepares, compounds, and accurately dispenses medications.
Interprets physician medication orders for accuracy and appropriateness, informs physicians of errors or omissions, and suggests appropriate alternative therapy when necessary.
Maintains accurate patient medication profiles, monitors drug selection, dosage, and interaction potential.
Dispenses, controls, and records all controlled substances.
Keeps a working knowledge of legal requirements, department policies, and procedures concerning specialty drugs.
Assumes charge pharmacist duties, coordinates pharmacists and support personnel, assigns work, delegates tasks, and handles issues during manager absence.
Supports pharmacists, managers, and directors to integrate departmental and organizational goals.
Supervises and trains pharmacy technicians, interns, and support team members; may precept Fellows, Residents, or PharmD student rotations.
Performs initial and recurring patient evaluation of prescribed therapies; collects, analyzes, evaluates, and documents data relative to medication therapy.
Communicates with medical staff to assist with medication‑related cost savings, clinical decision making, adherence interventions, and other aspects of medication therapy delivery.
Documents patient interactions in the EMR or other database per Specialty Pharmacy policy and procedures.
Completes intake process for new patient enrollment, insurance verification, medication refill, and delivery schedule.
Monitors specialty medication adherence and proactively addresses adherence concerns and barriers.
Complies with policy and procedures of pharmacy processes related to third‑party accreditation standards for specialty pharmacy.
Completes other responsibilities as assigned (data collection, reporting, process improvement, and patient education materials).
QUALIFICATIONS
Graduation from college with a degree in Pharmacy.
Advanced degree or enrollment in advanced degree program.
Minimum of one year experience as a registered pharmacist working with specialty medications in a hospital setting.
ASHP accredited residency in Specialty Pharmacy or related training.
Board certification or advanced credentials in area of practice.
Knowledge / Skills / Abilities
Excellent problem‑solving and communication skills (verbal & written).
Ability to work under pressure and be flexible in setting priorities.
Ability to do accurate, detailed analysis of complex information and issues is essential.
Possess extremely strong customer service skills.
Ability to effectively interact with team members and other departments.
Adaptable to changing priorities, tasks, and work schedules to meet customer service standards and needs.
License / Certifications
Possession of a valid license to practice pharmacy in the State of Minnesota.
