RN Care Manager – Institute on Aging Institute on Aging (IOA) is at the forefront of innovative healthcare models that enable aging adults to live in place. Responsibilities

Conduct comprehensive nursing health assessments of participants and develop individualized care plans (Plan of Treatment, POT).

Document assessments, case notes, care coordination, and utilization management within MedCompass.

Work collaboratively with participants, legal representatives, support circles, and primary care physicians.

Identify, implement, and monitor services within the POT, including community resources and public‑funded programs.

Coordinate and train backup caregivers for unpaid support when waiver service providers are unavailable.

Provide education, counseling, advocacy, and information to participants and their support systems.

Establish and maintain a care coordination schedule based on acuity and initial assessment.

Monitor delivery of HCBA Waiver services and quality of authorized services through ongoing contact, monthly visits, and telephone calls.

Conduct annual reassessments, care plan updates, and follow‑up after Emergency Department and inpatient facility admissions.

Travel throughout Riverside and San Bernardino County as required, including occasional cases outside assigned region.

Required Qualifications

Current, unencumbered license to practice as an RN in California.

Current Basic Life Support (BLS) certification.

Minimum 1,000 hours of acute‑care experience in the last two years caring for individuals meeting waiver levels of care.

Valid driver’s license and proof of insurance.

Willingness to travel in Riverside and/or San Bernardino Counties.

Experience establishing short‑ and long‑range goals for patient care within a continuum of care focus.

Experience with medical diagnoses and medication associated with functionally impaired adults and older adults.

Exceptional communication and presentation skills relating to functionally impaired adults and older adults, their support systems, and teams of health professionals.

Demonstrated case‑management experience and skills within the community health care delivery system.

Excellent detail orientation and problem‑solving skills; ability to prioritize multiple critical tasks.

Excellent computer literacy.

Desired Qualifications

Experience working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders.

Exceptional interpersonal, oral, and written communication skills.

Compensation Range: $100,000 - $107,000/annual
