Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. At CBI, a psychiatric practitioner plays a vital role in our whole person approach within our integrated system of physical, psychiatric, and addiction services. The scope includes seeing adults 18 and older, of all genders and backgrounds, with complex medical and psychiatric co-occurring disorders. Practitioners are expected to provide patient-centered, evidence-based care and serve as advocates in health-related matters, including specialist referrals, health services, and community resources. This document outlines the skills, activities, and expertise within the psychiatric practitioner’s scope of practice in inpatient and outpatient settings. Responsibilities include: Completing Medical Practitioner Admission Orders and CON (for physicians) and coordinating signing if not a physician Conducting psychiatric evaluations within 24 hours of admission and reviewing physical health H&P Reviewing and modifying treatment plans to meet psychiatric, medical detoxification, and physical health needs Participating in daily rounds with RN and Transition Coordinator for discharge planning Documenting daily rounds and monitoring treatment of physical health, coordinating with PCP and other prescribers Initiating or recommending consults and coordinating care Ensuring smooth transitions during discharges, admissions, or transfers, with proper documentation Providing documentation and treatment for court-ordered or petitioned patients, including Special Treatment Plans Signing discharge orders and follow-up recommendations within 24 hours Prescribing pharmacotherapy in line with standards of care, maintaining at least 80% e-prescribing, aiming for 100% Evaluating patients for addiction medicine protocols, Suboxone taper, ongoing mental health/psychiatric and physical health issues prior to discharge Universal requirements: Complete orientation, including shadowing Obtain and maintain TB skin test, Suboxone waiver, CPR/First Aid certification Participate in quality initiatives, maintain documentation compliance, and adhere to scheduling protocols Maintain patient satisfaction and follow-up rates, document assessments and consents timely Review controlled substance monitoring, utilize e-prescribing, and participate in team activities and meetings Support utilization review and management activities, communicate effectively with referral and receiving professionals Follow policies and procedures, provide timely notice for time off, and notify in emergencies Additional info:
