Overview
The Division of Nephrology at the University of Virginia is seeking a full-time Advanced Practice Provider (PA/NP/APP) to join our renal team in Orange/Zions Crossroad/Culpeper and/or Charlottesville, VA locations. The APP will provide comprehensive, specialized patient care in outpatient dialysis settings, including assessment, treatment planning/implementation, and education of patients and families with chronic kidney disease and end-stage renal disease. The APP practices independently in coordination with an interdisciplinary team and medical director to deliver quality healthcare across the lifespan. The role may involve acute dialysis coverage and/or ambulatory clinic depending on experience and interest. Practice hours: Monday – Friday, 8 am – 5 pm. Responsibilities
Outpatient Dialysis:
Round on ESRD and AKI-D patients in person a minimum of 3 times per month independently; assess, diagnose and manage ESRD complications (anemia, blood pressure and volume management, bone disease, fluid/electrolyte imbalances, vascular access); provide patient and family education promoting transition to home dialysis modalities and transplant; diagnose and manage episodic health issues; monitor and interpret laboratory tests, imaging, and other diagnostics; participate in interdisciplinary rounds monthly and quality task forces, providing clinical leadership. Clinical:
Independently and in collaboration with physicians and the interprofessional team, provide comprehensive clinical and organizational direction; perform comprehensive history and physicals; document findings; order and interpret diagnostic labs/tests; form medical diagnoses; develop, implement and evaluate treatment plans; prescribe pharmacological and non-pharmacological interventions; initiate specialty referrals and clinical pathways; collaborate with Attendings/Fellows on admissions and transfers; facilitate patient flow; recognize acute/emergent situations and initiate life-saving interventions; communicate plans with patients and families; advocate for patients’ education, treatment options, participation in care and ethical considerations; coordinate care standards with team; explain procedures; perform designated procedures after demonstrated competency. Educational:
Act as a clinical resource for education of the interprofessional team; provide education aligned with evidence-based practices; precept APP students and mentor other APPs; provide outreach and education to the community; coordinate educational standards with the team. Leadership:
Serve as a role model, demonstrate respect, mentor others, and assume leadership in developing the Advanced Practice role for patients, families, professionals, and the community; coordinate leadership standards with team members; meet leadership competency standards. Quality/Stewardship/Outcomes:
Participate in quality assurance initiatives, performance improvement and benchmarking; apply evidence-based practice to patient care; assess access, quality, safety and cost; help develop instruments to measure outcomes; identify needs and variations to improve quality; maintain regulatory documentation; contribute to clinical pathways and protocols; participate in accreditation preparedness and performance improvement standards. Research:
Pursue opportunities for active engagement in research and analysis of evidence-based practice; coordinate meeting applicable research standards. Professional Development:
Maintain licensure, state certification, and required practice agreements; maintain competencies and credentials for privileges; pursue educational opportunities (CEU/CME, conferences, seminars); participate in committees and present at meetings as appropriate. Minimum Requirements
Education:
Master’s degree as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant). Experience:
One to two years minimum as an APP (NP or PA) preferred. Licensure:
License or license-eligible as an APP in Virginia (Nurse Practitioner or Physician Assistant); NP must hold RN license in Virginia; eligibility for credentialing and licensure with UVA Medical Center, Virginia Board of Medicine or Board of Nursing, and certifying bodies; enrollment with payers through Delegated Credentialing; able to participate in Medicaid/Medicare billing; current BLS; PALS/NRP as required; ACLS as required. Job requires standing for prolonged periods; frequent travel/bending/stooping; proficient communication, hearing and vision; attention to detail; ability to lift/push/pull 20–50 lbs; potential exposure to chemicals, blood/body fluids and infectious disease.
