The Enrollment Specialist will coordinate and conduct new enrollments for participants into Neighborhood Healthcare's PACE (Program of All Inclusive Care for the Elderly) program. The Enrollment Specialist will focus on completed prospect intakes and enrollment conferences resulting in PACE program enrollments. Additionally, this position will collaborate with the Outreach Specialists team to increase census within the program.
Hybrid Schedule: Monday-Friday 8:00am-5:00pm.
In addition to a competitive hourly rate, this position offers both individual and team performance bonuses, providing excellent earning potential.
Responsibilities
Conducts and completes program eligibility screenings, intakes, assessments, education, and enrollment assistance activities with potential participants and/or their families to determine eligibility for PACE
Coordinates communications of eligibility decisions, approvals, or disapprovals with participant or their family members
Participates in required meetings, such as the interdisciplinary, (IDT) management and other related meetings and activities to discuss participant enrollment and outreach progress and efforts
Works with Outreach Department on community based activities to attract potential participants at events at sites and in the community
Collects and enters information related to referrals, intakes, enrollments, marketing, and outreach activities notes for weekly and monthly reports for PACE and Marketing
Provides program tours, intakes, education, and enrollment assistance to determine participant needs and enrollment eligibility, including in their participants’ homes, as needed
Facilitates referrals for participants for assistance from Neighborhood and other community resources
Provides responsive and high-quality services to patients, staff, and outside agencies
Impacts patient and team experience by demonstrating courteous and helpful behavior and a commitment to accuracy
Provides input and feedback on PACE’s internal and external outreach, education, and enrollment plans for engaging, assisting, enrolling, and retaining new and current participants
Conducts community outreach and education activities to patients and community members at clinic locations and other community locations about PACE and other related health coverage options
Adjusts outreach and enrollment strategies to align with trending data analysis for better engagement rates with participants
Maintains effective interpersonal relationships with all levels of staff, other departments, agencies, providers, and the public
Performs outreach and enrollment activities according to Neighborhood and outside agencies policies & procedures
Qualifications Education/Experience
High school/GED required; bachelor’s degree in health or human service preferred
One year of community health, senior health, marketing, or health/services related enrollment experience required
One year of Medi-Cal, Medicare, managed healthcare eligibility, or senior related sales experience preferred
Valid CA driver’s license and proof of auto insurance required
Must take and pass the California Department of Health Care Services (DHCS) and Medi-Cal Marketing exam within 60 days of hire
Bilingual(Spanish) highly preferred
Additional Qualifications (Knowledge, Skills and Abilities)
Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
Knowledgeable about and experience with frail or elderly populations
Skilled in applying sales and marketing strategies to effectively engage and enroll individuals from diverse social and ethnic groups, while building trust and generating referrals
Experienced in using systems and processes to accurately track, measure, and analyze sales activity to drive enrollment goals
Proficient in developing new contacts and cultivating strong referral networks to support sustained enrollment growth
Adept at tailoring outreach and marketing approaches to connect with diverse communities and convert interest into successful program enrollments
Ability to successfully manage multiple tasks simultaneously
Excellent planning and organizational ability
Ability to work as part of a team as well as independently
Ability to work with highly confidential information in a professional and ethical manner
Physical Requirements
Ability to lift/carry 15 lbs/weight
Ability to stand for long periods of time
Neighborhood Healthcare offers a generous benefit plan that includes: Partially company paid Medical, Dental, and Vision Plans. Pay range: $29.71 - $40.96 per hour, depending on experience. In addition to a competitive hourly rate, this position offers both individual and team performance bonuses, providing excellent earning potential.

The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
