to meet with clients in their homes and communities. About Us In 1984, the founders of the Access Support Network (ASN) witnessed an urgent need in the community to provide advocacy and support for people living with HIV/AIDS. Since then, the ASN has grown and adapted to the community’s unique needs. In 2001, the SLO Hep C Project was developed to support community members living with hepatitis C. In 2014, the ASN became the first Covered California-certified enrollment entity in San Luis Obispo, helping to ensure that the community had access to affordable health insurance programs. Thanks to the strength and support experienced in SLO County, the ASN expanded into Monterey County in 2015, then Santa Barbara County in 2020. Services have also expanded to reflect the agency’s belief in harm reduction as a life-saving tool that prevents disease and drug overdose. The ASN remains the sole community-based, non-profit organization in San Luis Obispo, Monterey, and Santa Barbara Counties that provides supportive services to residents living with HIV/AIDS and Hepatitis C and their loved ones.The agency strives to improve its clients’ quality of life and independence by providing individual support and meeting the unmet needs of our community members. To learn more, visit www.asn.org Responsibilities ●Provide client-centered case management for individuals living with HIV/AIDS. ●Assess housing, medical, financial, and psychosocial needs. ●Develop and monitor individualized Housing and Service Plans (HSPs). ●Assist clients in locating, securing, and maintaining housing. ●Help clients apply for benefits such as HOPWA and other housing-related services. ●Maintain regular communication with clients in-person and virtual check-ins. ●Collaborate with landlords, housing agencies, and medical providers. ●Ensure accurate documentation in agency databases and client files. ●Participate in case conferencing, training, and team meetings. ●Respond to housing and client emergencies as needed. Requirements ●Bachelor’s degree in Social Work, Psychology, Public Health, or related field (or equivalent experience). ●Experience working with individuals impacted by homelessness, HIV/AIDS, substance use, or mental health challenges. ●Understanding of housing resources, benefits programs, and social services. ●Strong organizational, communication, and interpersonal skills. ●Ability to work independently and travel regionally weekly. ●Valid driver’s license and reliable transportation. Preferred Qualifications ●Familiarity with HOPWA, HUD guidelines, or Ryan White programs. ●Experience with Harm Reduction and Trauma-Informed Care practices. ●People with Lived Experience are encouraged to apply. ●Competitive salary (DOE) ●Health, dental, and life insurance ●Paid time off and holidays ●Employee Assistance Program ●Office and field-based work and remote ●Frequent travel within the assigned region ●Occasional evening or weekend hours may be required Join Us!
