Administrative Functions: Administer patient assessments, oversee the assessment process, set assessment schedules, and assure assessments are done accurately and in a timely manner. Coordinate the care plan according to regulatory requirements. Create the schedule for all Medicare and Medicaid. Start Medicare coverage for newly qualified patients or send out denial letters. Stay updated on changes in Medicare coverage and help determine documents needed for Medicaid reimbursement. Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility. Ensure nursing personnel comply with the facility\'s written policies and procedures. Periodically review the department\'s policies, procedure manuals, job descriptions, etc., and make recommendations for revisions. Manage shifts: meet with assigned nursing staff and support personnel to plan shifts\' services, programs, and activities. Ensure the Nursing Service Procedures Manual is current and reflects day-to-day nursing procedures. Ensure compliance with procedures; make written and oral reports/recommendations as required. Coordination and reporting: Cooperate with other resident services to ensure the resident\'s total regimen of care is maintained; participate in the facility\'s quality assurance program; participate in surveys; review discharge plans; assist in updating and planning the nursing services portion of the resident\'s discharge plan; interpret policies to personnel, residents, visitors, and government agencies. Admit, transfer, and discharge residents as required. Complete accident/incident reports as necessary. Write resident charge slips; maintain the Daily Census Report; perform administrative duties such as completing medical forms and charting; protect resident information and report violations as required. Charting and documentation: Complete and file required recordkeeping forms upon admission, transfer, and discharge. Encourage physicians to review and sign orders and notes. Receive and transcribe physician orders; chart nurses\' notes; document incidents; manage diet orders and communication with Food Services. Record and report discrepancies and transfer information per procedures. Ensure documentation of unauthorized discharges and routine charting in accordance with policies. Drug administration: Prepare and administer medications as ordered; verify resident identity; maintain stock; order medications and supplies as needed; ensure narcotic records are accurate; notify discrepancies; review medication cards; communicate automatic stop orders; dispose of drugs per procedures. Personnel functions: Participate in evaluations and staffing decisions; identify and address staffing needs; provide leadership; conduct rounds; ensure staff are trained; monitor compliance with policies; foster interdepartmental collaboration; promote a positive unit culture; address complaints and grievances; follow disciplinary procedures; manage shift handoffs and report exposures and security incidents. Nursing care functions: Inform and prepare rooms for admissions; orient residents and families; coordinate diagnostic and therapeutic services; consult with physicians; implement nursing objectives and standards; ensure proper administration of care and treatments; supervise CNA performance and adherence to care plans; coordinate social and activity programs; communicate changes in resident condition; implement restorative and rehabilitative programs; ensure cleanliness and safety of treatment areas; perform professional procedures as needed; use restraints per policy; collect specimens; monitor vitals; manage meals and dietary adherence; check and communicate dietary issues with Dietitian. Care planning and assessment: Review care plans daily; update care plans as needed; ensure CNAs follow plans; assist in planning, scheduling, and revising the MDS and RAPs/Triggers; ensure CNAs refer to care plans before daily care. Budget, planning, and miscellaneous: Report fraud related to false billings and cost reports; provide data to QA/assessment committees; manage and plan departmental operations. Safety, infection control, and equipment: Monitor safety regulations; ensure PPE availability; maintain clean and sanitary work areas; enforce hand hygiene and fire/disaster drills; participate in infection control programs; ensure safe operation of equipment; manage hazardous materials and MSDS documentation. Care team leadership: Create an atmosphere of warmth and teamwork; develop good rapport with other departments and staff; promote high-quality resident care; participate in training and development; ensure staff coverage and scheduling; supervise and mentor nursing personnel. Other duties: Note that duties may be reassigned by management as needed.
Qualifications
Education and/or Experience: Nursing Degree from an accredited college or university, or graduate of an approved LPN/LVN program; current, active license to practice as an LPN/LVN in this state; CPR certification; knowledge of PCC preferred; continuing education/licensing maintained; good standing with State Board of Nursing. Language: Must read, write, speak, and understand English; ability to read technical procedures and policy manuals; ability to present information to managers and staff. Mathematical: Ability to apply fractions, percentages, ratios, and proportions. Reasoning: Ability to provide age-appropriate nursing care, work well in a team, make independent decisions when required, and interact tactfully with residents, families, and staff. Knowledge of nursing practices, regulations, and laws relevant to nursing facilities; leadership and supervisory ability; planning and program interpretation abilities; patience and a proactive attitude. Certifications: Current LPN/LVN license; CPR; PCC knowledge preferred; maintain continuing education/licensing; good standing with State Board of Nursing. Physical Demands: Ability to move intermittently; read/write English; handle mental and emotional stress; vision and hearing capabilities; ability to lift at least 25 pounds to 5 feet and 50 feet distance; able to assist in evacuations. Work Environment: Office and nursing areas; frequent interruptions; potential exposure to infectious materials and chemicals; subject to shift work, emergencies, and long hours; ability to respond to hostile or upset individuals; willingness to participate in ongoing education.
