Certified Recovery Mentor
offers case management, including referral linkage, individual counseling, family support, group counseling, and case management services to support clients' engagement in treatment and help families support their continued recovery. Location:
This position is based at LifeWorks NW's Mountaindale site. There is a $1.25 location differential for this position. ESSENTIAL RESPONSIBILITIES: Screens and selects clients for the mentoring program. Develops service plans for clients, including referral reporting. Within five days of intake, meets with the client and family (if appropriate) to develop a case management plan. Maintains daily contact with clients for up to five weeks post-release to assist with recovery support, housing, employment, family adjustment, legal, medical, and benefit issues. Contact frequency decreases to twice monthly or monthly by day ninety. Assists families in implementing the plan, maintains communication with therapists, collateral providers, community corrections, and links clients to community services. Determines when to terminate clients from the program due to service completion or lack of response. Documents and maintains client records in the electronic healthcare system within 24 hours of meetings, ensuring confidentiality and compliance. Collaborates with social service agencies to update services, review referrals, and facilitate treatment planning. Travels between sites or in the community to support clients as needed. REQUIREMENTS: High school diploma or GED. Current Certified Recovery Mentor Certificate (CRM) with MHACBO. Experience with addiction issues, including cultural and linguistic nuances. Ability to maintain confidentiality and appropriate boundaries. Knowledge of navigating social service networks. Ability to accompany clients in their environment. Knowledge of legal and corrections systems. Effective communication skills and cultural competence. Availability to work some evenings and weekends. Valid driver's license, auto insurance, and a safe driving record.
