The

Housing Substance Use (SU) Case Manager (CASAC)

provides client-centered substance use counseling and case management to individuals enrolled in the HUD Connection to Housing & Care Program. This role is vital in helping clients achieve housing stability while addressing substance use challenges through individual and group counseling, advocacy, and supportive services.

The SU Case Manager conducts intakes, home visits, assessments, treatment planning, and ongoing support, ensuring that clients receive the resources, care, and services needed for a stable and healthy life. This position also plays a crucial role in reducing barriers to care, connecting clients with medical, mental health, and housing services, and advocating for their needs.

Essential Job Functions

The following duties are mandatory requirements of the job:

Client Engagement & Support

Caseload Management: Maintain a caseload of up to twenty-five (25) clients, including single individuals and families across the five boroughs of New York City. Home Visits: Schedule and conduct at least one monthly home visit per client to assess housing stability and provide case management services. Build and maintain trusting relationships with clients to support their journey toward recovery and stability. Conduct screenings and intake assessments to evaluate client needs, substance use severity, and readiness for care. Provide individual and group substance use counseling to help clients navigate challenges and develop coping strategies. Develop personalized treatment plans that outline short- and long-term goals for recovery, counseling, and supportive services. Conduct monthly home visits for clients residing in New York City to assess progress, provide in-person support, and ensure housing stability. Support clients in understanding and adhering to housing program requirements, including the Terms and Conditions of Tenancy.

Case Management & Advocacy

Offer comprehensive case management, including referrals to medical care, mental health services, housing support, and community resources. Connect clients to GMHC services and coordinate with external providers for additional care as needed. Assist clients in maintaining housing permanence by monitoring and facilitating monthly rent payments. Escort clients to healthcare providers, entitlement agencies, and housing management offices to ensure access to necessary services. Conduct hospital and off-site visits to maintain engagement with clients in crisis or undergoing treatment. Advocate for clients in case conferences with housing providers, medical teams, and other service providers.

Compliance & Administrative Responsibilities

Ensure accurate and timely documentation of all services, including: Quarterly service plans and ongoing assessments. Monthly progress notes reflecting client progress and engagement. Completion of monthly home visit documentation. Data entry of contract deliverables into eCOMPAS, AWARDS, and other tracking systems. Monitor and meet all contract service deliverables, including home visits, advocacy efforts, and counseling sessions. Maintain confidentiality and compliance with state and federal regulations related to client care and substance use treatment. Attend GMHC meetings, training sessions, and organizational events to stay updated on best practices and agency policies. Perform other duties as assigned by the supervisor to support program goals and client well-being.

Education and Certification

High School Diploma, GED, or HSE required. Current CASAC (Credentialed Alcoholism and Substance Abuse Counselor) license required.

Special Skills and Knowledge

Experience providing substance use counseling and case management in a community or healthcare setting. Strong ability to work in a fast-paced, data-driven, and performance-based environment. Excellent communication, problem-solving, and interpersonal skills to engage with clients and service providers effectively. Ability to work independently and as part of an interdisciplinary team.

Preferred

Bilingual in English and Spanish strongly preferred. Experience working with individuals living with HIV/AIDS, substance use disorders, and/or mental health conditions. Knowledge of housing stability resources and rental assistance programs. Experience working with LGBTQ+ communities, historically marginalized populations, and nonprofit organizations.

Technical Skills

The ideal candidate should have the following technical competencies to effectively manage case documentation, reporting, and client services:

Intermediate Proficiency In Microsoft Excel, Including

Data tracking and analysis for case management and program reporting. Creating and managing spreadsheets to monitor client progress and deliverables. Using formulas, pivot tables, and data validation to ensure accuracy in documentation.

Basic Proficiency In Microsoft Office Suite, Including

Outlook (email communication, scheduling, and coordination with service providers). Word (creating case notes, service plans, and official correspondence). PowerPoint (preparing presentations for internal and external stakeholders).

Experience with electronic case management and data tracking systems, such as:

eCOMPAS, AWARDS, or similar platforms for documentation and compliance reporting. Data entry and management to ensure contract deliverables are accurately recorded.

Familiarity With Government Benefit Systems And Entitlement Programs, Including

Social Security, Medicaid, SNAP, and Housing Assistance Programs. Ability to navigate online portals and submit required client documentation.

Strong Documentation And Compliance Skills, Ensuring

Accurate case notes and service plans in adherence to regulatory requirements. Confidentiality and compliance with HIPAA and other legal protections.

Work Environment & Schedule

This Is a Hybrid Position Requiring a Mix Of

Fieldwork (conducting home visits, client escorts, and community outreach). In-office duties (team meetings, case conferencing, and reporting). Remote work as determined by the supervisor based on program needs. Participation in the annual AIDS Walk New York event is mandatory for all GMHC staff.

While Carrying Out The Responsibilities Of This Role

Employee is routinely required to sit, engage in conversation, actively listen, and take notes. Frequent travel across the five boroughs of New York City using public transportation. Additionally, the employee must regularly stand, walk, and navigate stairs.
