Advanced Practice Provider (NP/PA) - Heart Failure (rotational) role at

The Ohio State University Wexner Medical Center . The APP provides health care services to patients within the parameters of a standard care or supervisory agreement with an attending physician member of The Ohio State University Wexner Medical Center. Collaborates with a physician and other clinicians in providing a full scope of patient care. The focus is on preventive health or the management of acute and/or stable chronic conditions. Performs medical services that have been specifically authorized and directed by the credentialing body and the certifying organization through which they hold licensure. Provide health care services to patients working in collaboration with a licensed physician member of the medical staff. Care during acute and chronic phases of illness, education, and counseling of individuals and families, health promotion and prevention, and referral to other healthcare providers and community resources when appropriate. Service line was started in 2018, to see all Advanced Heart Failure patients, pre- and post-transplant, LVAD, inotrope, and admit patients with Heart Failure that have other acute medical conditions. Qualifications

Minimum Qualifications :

Required : Master\'s of Science Degree or DNP in Nursing or Master\'s in Physician Assistant Science. APRN License with Certification as an NP or PA-C. 0 years of relevant experience required. Preferred : 0-2 years of relevant experience preferred. Additional Information
