Position Summary
Provide individual counseling, group counseling and case management services to individuals with Substance Use Disorders (SUD) so they may achieve sustained remission from SUD, restore and increase healthy social connections, obtain stable income and permanent housing, and reduce their risk of homelessness. As a Level III Case Counselor also conduct diagnostic assessments, participate in Quality Assurance activities, supervise interns, provide Level II Case Counselors with mentoring and support, and provide group supervision to clinical staff as needed. Essential Functions
Provide individual counseling, outpatient group counseling, intensive outpatient group counseling, and case management to homeless individuals with substance use disorders. Provide individual counseling and case management to a case load of program participants. Facilitate Outpatient Group Counseling and Intensive Outpatient Group Counseling. Record all service activities by entering progress notes in the Y-Haven Electronic Medical Record. Establish and maintain regular collaborative relationships with social service organizations and programs that residents may utilize for employment, housing, continuing recovery and other needs. Participate in daily team meetings, quality assurance meetings, in-service trainings and coordination meetings. Coordinate with Clinical Director on all critical clinical issues. Education Master's Degree or enrolled in Masters program in relevant field (Required) Experience Must have at least 3 years' experience in SUD Counseling. Certification(s) Minimum License: LCDCIII (Licensed Chemical Dependency Counselor III), LSW (Licensed Social Worker), LPC (Licensed Professional Counselor). CPR and First Aid certification and Safety training required within 30 days of hire. Must be free from sanctions or exclusions from the Office of the Inspector General (OIG) and the State of Ohio Department of Medicaid. Technical Skills and Knowledge
Bilingual (Spanish/English) preferred, but not required Proven ability to meet minimum productivity requirements. Must have successful experience facilitating SUD group counseling. Must have extensive knowledge of Substance Use Disorders and other issues affecting the homeless population. Must have working knowledge of OHMAS, CARF and VA program compliance standards. Demonstrate commitment to the mission, cause of the YMCA, and uphold its values and ethics. Ability to work effectively with people of different backgrounds, abilities, opinions and perceptions Willingness and ability to be a mandated reporter for any signs or suspicion of child abuse and neglect. All employees are required to follow the child abuse and neglect reporting process: completion of Suspected Child Abuse/Neglect reporting form, calling children protective services (696-Kids) and notifying your immediate supervisor, branch executive, risk management and human resources department(s) Work Environment
