Summary Under general direction, the Case Manager will coordinate existing services for the unhoused in areas such as prevention, early intervention, and support services. Establishes, maintains, and enhances cooperative relationships with local service providers and the unhoused community. Evaluates the social needs of each client and formulates a written plan of care. Refers clients to appropriate community resources as necessary.
Essential Duties And Responsibilities
Provides Case Management to persons experiencing homelessness
Serves as a liaison to the unhoused community and coordinates services
Meets with prospective clients to determine eligibility and sheltering needs
Handles telephone calls and interviews each client on admission
Provides a presence and monitors unhoused campsites throughout the City
Conducts interviews with campers and those who are in need of services to better assist in coordinating social service
Plans and organizes social service program according to established policies
Assists in the coordination of resident transfers
Connects clients with appropriate agencies and services
Collects information from each client on the social history
Identifies personal, emotional, and environmental concerns
Formulates a Care Management Plan for each client to include housing, employment, physical health, mental health
Reviews and updates Care Management Plan regularly with the client
Documents and records progress on the client's social needs and delivers response to social services
Assists client to identify resources from other professionals or agencies to solve problems
Helps clients locate funds for dentures, glasses, etc.
Participates in case conferences with agencies, nurses, and other resources; assists to understand the client’s needs
Follows the city policies, procedures, and safety guidelines
Performs other duties as assigned
Minimum Qualifications
Combination of education and experience equivalent to a Bachelor’s degree in social work, public health, or related area
Two years of experience in the field of social services with unhoused or low-income services or related case management area
Preferred Qualifications
Experience working with diverse agencies and community groups in a local government or non-profit agency
Experience in Community outreach, advocacy, and public education
Certificates, Licenses, Registrations
Valid driver’s license
