Job Summary : Provide skilled therapy intervention and treatment to patients in the home setting. This includes appropriate utilization of treatment plans and assessment skills, careful monitoring of patient status and progress toward goals, and compliance with physician's orders. Responsible for educating patients and caregivers, and preparing the patient/caregiver for discharge as appropriate.

(a) (1) Is licensed or regulated as an Occupational Therapist by the State, unless licensure does not apply;
(2) Has graduated from an accredited Occupational Therapist education program recognized by ACOTE of AOTA or successor organizations;
(3) Is eligible to take or has successfully completed the NBCOT certification exam.
(b) For those licensed or regulated before December 31, 2009, or outside the US after January 1, 2008, specific criteria are outlined regarding licensure, graduation, and certification.
Responsibilities/Essential Functions : The individual must perform these functions with or without reasonable accommodations:
Implement occupational therapy services as per physician’s plan of treatment.
Assist in evaluating and assessing the client's functioning and help develop and revise the care plan.
Use therapeutic activities to restore function and support self-care activities.
Document and report client reactions and condition changes to the physician and care team.
Educate clients, caregivers, and healthcare team on therapy phases, adaptive devices, and equipment.
Provide staff in-service education as directed.
Assess and re-evaluate client needs, and develop and revise care plans.
Initiate appropriate diagnostic, preventive, and rehabilitative procedures.
Assist with care plans and ensure continuity of multidisciplinary care.
Promote care coordination through meetings and communication.
Maintain accurate client care records and documentation.
Stay updated on occupational therapy trends and knowledge.
Participate in staff meetings, in-service programs, and quality improvement initiatives.
Supervise the Occupational Therapy Aide at least every 30 days.
Work Environment and Physical Requirements
Physical Elements : Clear communication, good observation, manual dexterity, mobility, and physical reflexes to perform job duties.
Environmental Elements : Work in office and patient home environments, which may vary in cleanliness and temperature, with potential exposure to hazards. Travel to various patient homes is required.
