Responsibilities
Responsible for promoting appropriate Length of Stay (LOS) and resource management in areas assigned; assesses the care needs of patients upon entry into the organization and facilitates appropriate progression through the hospital's continuum of care into the post-hospital environment; collaborates with other members of the health care team in planning, coordinating, and facilitating the implementation of a plan of care based on the reason for admission and current, emerging and projected care needs; performs these duties in a manner which enhances service excellence including quality clinical outcomes for patients, efficiencies in hospital operations, and cost-effectiveness; and demonstrates accountability for services provides through participation in the monitoring and evaluation of patient care activities. Other duties as assigned.

Communication and interpersonal skills, basic computer skills, and customer service skills required.

Qualifications
A bachelor's degree in a health care related curriculum is preferred. Candidates must possess at least a bachelor's degree from an accredited program.
A minimum of three (3) years’ experience in direct patient care in an acute care setting; additional experience in Case Management/Utilization Management/Discharge Planning is required.
Nurse candidates must hold a current licensure as a Registered Nurse in the state of Florida.
BLS is recommended, but not required.
