Brockton Home Health Care Agency is currently looking for an intake coordinator to manage the referral and registration process for new patients, acting as the first point of contact for patients, caregivers, and healthcare providers. Key responsibilities include assessing patient needs, gathering clinical and insurance information, verifying benefits and authorizations, coordinating services with physicians and payers, and scheduling initial visits to ensure a smooth transition to home care.

Brockton Home Health Care provides health services, skilled and non-skilled, to adults and elderly with home support. Brockton Home Health Care also provides health care staffing for health facilities and non-medical transportation services.

Key Responsibilities

Manage and review the initial assessment of referrals, gathering necessary clinical and demographic information from patients and referral sources.

Follow up with the case manager from the referral source to communicate if the referral was accepted or denied.

Maintain tracking of admissions, discharges, and hospitalizations.

Respond to incoming calls and inquiries from physicians and other referral sources, determining patient eligibility for home health services.

Coordinate with the clinical scheduling teams, schedule initial home health visits, and ensure appropriate clinicians are assigned to cases.

Provide a weekly report of number of referrals received and number of referrals accepted to leadership.

Serve as a primary point of contact for patients, families, physicians, and other healthcare providers, providing information and support.

Enter patient information into electronic health records (EMR) and maintain accurate logs of all intake activities and patient data.

Qualifications

The ideal candidate has previous experience in a customer service or administrative role.

Strong organization skills, ability to multi-task, and capacity to manage time to meet changing deadlines in a fast-paced environment.

Excellent communication, customer service, and problem-solving skills, including the ability to maintain composure under stress.

Excellent telephone skills.

Highly professional and dependable.

Proficient computer and internet skills, including Microsoft Office suite.

Previous customer service or administrative experience required; health care and/or home care experience preferred.

High school diploma or GED required.

Preferred Associate degree.
