GENERAL SUMMARY: As a Community Health Navigator, you will play a key role in assessing community health needs, guiding individuals to relevant resources, and strengthening connections between our hospital and the community. Your collaborative efforts with healthcare providers and local organizations will be instrumental in assessing health needs ensuring seamless access to essential services. Collaboration with healthcare providers, community organizations, and local agencies will be central to your role in promoting comprehensive health solutions. These duties are to be performed in a highly confidential manner, following the mission, values, and behaviors of Insight Hospital and Medical Center. Employees are further expected to provide a high quality of care, service, and kindness toward all patients, staff, physicians, volunteers, and guests.
DESCRIPTION: Duties and Responsibilities Assist individuals in navigating healthcare systems, ensuring access to appropriate services. Collaborate with interdisciplinary teams to develop and implement community health initiatives. Maintain accurate records and reports to assess the impact of interventions and measure outcomes. Establish and nurture relationships with healthcare providers and community organizations. Conduct community assessments to identify health-related challenges and gaps. Provide health education and outreach programs to raise awareness and promote preventive care.
JOB SPECIFICATIONS: REQUIRED KNOWLEDGE, SKILLS AND ABILITIES Knowledge:
An Associates, bachelor's or higher in a related field (e.g., Public Health, Social Work). In-depth knowledge of Insight Hospital's community health resources and services. Proven experience in community outreach or health navigation. Skills:
Understanding of public health principles and social determinants of health. Familiarity with healthcare systems, insurance, and patient advocacy. Proficiency in data collection, analysis, and reporting. Preferred to have and maintain a driver’s license. Abilities:
Ability to assess and address diverse needs of community members within the hospital's service area. Outstanding rapport-building and customer service skills with an ability to work with diverse groups and income levels. Excellent organizational and problem-solving abilities.
