Position Summary
The Case Manager participates in the development of comprehensive assessments, service plans, case conferences, discharge planning, referrals, advocacy, and linkages to needed services and supports. The case manager acts as a bridge between patients, their families, and school and state partners, ensuring comprehensive and coordinated care. The Case Manager facilitates group programming through evidence-based curriculums, and provide youth and/or family supportive counseling. Full Time: 40 hours per week, flexible schedule.
Responsibilities
Provide case management: facilitate group programming through evidence-based curriculums, provide youth and/or family supportive counseling, and provide referrals, advocacy, and linkages
Participate in the development of service plans, comprehensive assessments, discharge planning, and case conferences
Coordinate interventions and services provided by multiple agencies and school districts including within CFS programs
Complaint with funding source, contract and or program's Annex A
Complete required documentation within established timeframes per Federal, State, County, and agency regulations, policy and procedure
Appropriately utilizes supervision process and agency supports
Participate in team meetings regarding the youth's behaviors and ongoing service needs
Collects statistics and completes written reports as required
May provide specialized activities and functions specific to program
Qualifications
Bachelor Degree in Human Services, Social Work, Sociology/Psychology from an accredited institution with 3 years relevant experience OR
Unlicensed Master's Level practitioner with 1 year of related experience.
Work Environment & Physical Requirements
Ability to sit for extended periods during documentation tasks, group meetings, and administrative tasks.
Ability to walk up and down stairs.
Capability to stand for prolonged periods during presentations, events, or meet-and-greet sessions.
Ability to assist youth with daily activities and participate in group recreational or therapeutic activities.
Ability to meet and fulfill all physical requirements of Handle with Care Training
Be comfortable working in various residential settings, including shared spaces and therapy rooms.
Ability to travel to various locations, including family homes, community organizations, and program sites, using reliable transportation.
Capability to navigate staircases, uneven terrain, and other physical environments encountered during community visits
Provide assistance in the use of adaptive equipment and assistive devices for youth with specific needs.
Ability to use a computer, keyboard, and other office equipment with precision.
Visual acuity to review documents, input data, and perform detailed tasks.
Adequate hearing and speech capabilities to engage in conversations, presentations, and phone communication clearly and effectively.
