Overview
The Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to assigned populations. The ACN uses a collaborative approach to anticipate, integrate, and coordinate the patient’s plan of care to help patients remain in a community setting. In collaboration with physicians, the nurse identifies at-risk patient populations requiring care management services. The ACN acts as a patient/family advocate and serves as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources. The Ambulatory Care Nurse understands organizational needs and supports the mission, values, and management of patient care services. The ACN actively incorporates ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.
