RN Case Manager – Ontario County

Base pay range $70,000.00/yr – $97,500.00/yr
Role and Responsibilities
Provide and coordinate quality patient care, maintain a full case load, meet productivity goals, and document accurately and timely.
Manage each patient’s care efficiently and cost‑effectively, achieving positive outcomes regardless of payer.
Oversee the overall plan of care and outcomes, including care delivered by other nursing staff (LPN, HHA) supervised by the RN Case Manager.
Conduct admissions and manage patient care: obtain medical history, perform physical examination, assess mental status, diet, ADL, and evaluate patient environment.
Determine necessary nursing and agency services, including home health aide services.
Interpret nursing and other agency services for patients and families, providing care planning and regulatory compliance.
Practice accepted nursing standards with regard to infection control.
Develop and implement nursing care plans, provide skilled nursing care as ordered, and perform preventive and rehabilitative procedures.
Document services and care plan revisions per standards of nursing practice.
Supervise home health aides and evaluate their care, maintain accurate records, and close discharge charts.
Other duties as assigned.
Education Requirements
Associate’s degree in nursing required.
Bachelor’s degree in nursing preferred.
Qualifications and Requirements
Current, valid RN license issued by NYS Department of Education required.
Minimum one year home‑care experience.
Knowledge of state and federal regulations, payer guidelines, and insurance requirements.
Excellent communication and critical‑thinking skills.
Flexible and organized.
Ability and desire to serve as a role model.
Working Conditions
Medium work: Exerting up to 50 pounds of force occasionally; occasionally lifting, carrying, pushing, and pulling objects.
Work performed in a non‑office setting; may involve outdoor exposure.
Frequent sitting and standing.
Physical Requirements
Stand, sit, hear, walk, talk, stoop or kneel.
Perform repetitive motion tasks.
