JOB SUMMARY The Outreach Housing Specialist coordinates assessments for individuals and families with homelessness needs, connecting them to shelters, rapid re‑housing, permanent supportive housing, and essential health and social services. The role includes participating in homeless coalition meetings, maintaining client data in HMIS and other required systems, and ensuring effective transitions to after‑care case management. The Specialist works closely with landlords, case managers, and community partners to secure and sustain permanent housing for clients.
ESSENTIAL RESPONSIBILITIES
Serve as a liaison, coordinator, and advocate between homelessness, housing case management, medical care, and community service providers.
Accompany clients to housing, medical, mental‑health, and substance‑use treatment appointments to improve housing stability and care.
Interview prospective clients to determine homelessness need, eligibility for services, and enroll them in community programs.
Maintain organized tracking of client outcomes through HMIS and follow‑up to monitor progress.
Conduct initial intake, housing barrier assessments, and document eligibility information for case plans.
Assist clients with housing searches, identify desired neighborhoods, and negotiate with landlords on behalf of participants.
Organize research, outreach, and public relations to build a pool of landlords willing to rent to program participants.
Support medical treatment coordination and follow‑up.
Maintain an active caseload, process and submit housing applications, and enter client data according to agency guidance.
Provide billing support and reconcile services across data systems such as PCIS, CASEWATCH, HIMS, and client electronic health records.
Participate in staff training and agency developmental activities as requested.
QUALIFICATIONS
Two (2) years of related work experience required.
Knowledge of homeless community resources, services, and the homeless continuum of care.
Experience working with multicultural and diverse populations.
Experience in Rapid Re‑Housing, Permanent Supportive Housing, and FQHC programs.
PHYSICAL REQUIREMENT Continuous sitting, walking, hearing, speaking, and occasional driving, stretching/reaching, standing. Work is primarily performed in an office setting.
TRAVEL REQUIREMENTS Own transportation required for home, airport, street visits, and annual commitments. Some weekend work may be requested.
