This position is responsible for conducting face‑to‑face assessments of members who may need skilled nurse assessment services. The incumbent completes all assessments using a skilled care acuity tool and provides a recommendation for services based on members' health care needs. Members who are assessed reside in the community or are ready to be discharged from a facility.
Base pay range $72,000 – $90,000 per year
Responsibilities
Perform face‑to‑face and/or virtual assessments of persons with disabilities.
Collaborate with health care providers and ancillary service providers to create or validate the appropriate treatment plan.
Document assessment results and negotiate service plans as needed.
Complete quality assurance competency reviews and other tasks/projects such as new assessor training.
Provide ongoing education and communication with health care providers to ensure appropriateness of care.
Serve as mentor or preceptor, assisting in formal training of assessors, completing QA audits, inter‑rater reliability reviews, and contributing to process improvement initiatives.
Qualifications
Four‑year nursing degree and at least three years of nursing experience.
Experience working with individuals with disabilities, intellectual and developmental disabilities (IDD), and pediatric or geriatric populations with complex health needs preferred.
Current and unrestricted RN licensure in Colorado or a compact nursing license; bilingual or Spanish speaking preferred.
Expected to have occasional travel to surrounding areas.
