Job Summary
This position is responsible for providing HIM support in maintaining the Department’s paper and electronic health record system. The HIM Specialist Sr will have an understanding of privacy and security guidelines related to patient information and maintain HIPAA as well as keep all patient information confidential. The key duties of this position rely heavily on the ability to learn and navigate efficiently within an electronic health record. The incumbent must be able to communicate effectively in writing and orally, work in a fast‑paced environment, and develop sound professional working relationships. This position is responsible for advanced functions within the Health Information Management Department.
Essential Duties and Responsibilities
Document Imaging (Pages/Hour)
Prepping, Scanning, Quality Review/Validation/Indexing
Perform functions assigned by HIM Leadership, including prepping, scanning, and QX/Indexing within Document Imaging to ensure records are prepped, scanned, and QC/Indexed into the Document Management Solution/EMR on time.
Scan and upload medical records into the document management application (VitalChart ChartFlow, formerly EvriChart) for remote coding access, including data related to prepping records to scan.
Document Chart Analysis: Analyze medical records for completion; assign and reassign provider deficiencies as needed in accordance with regulatory guidelines and facility completion policy.
Support Missing Documentation and Provider Queries as needed.
Complete birth and death certificates in accordance with state law, contacting parents within the required timeframe.
Chart Pick Up (Rounding) and Reconciliation.
Perform Data Integrity duties (chart correction, duplicate medical record, etc.).
Meet productivity expectations based on job function (document imaging / document analysis).
Maintain productivity metrics and time monitoring.
Assist internal and outside agencies with patient record requests, ensuring prioritization.
Answer phones, process continuity of care requests, record retrieval, and record filing as assigned.
Other duties as assigned.
Knowledge, Skills, and Abilities
PC Skills – proficiency in keyboard operations and Microsoft Office applications.
Customer Orientation – establish and maintain long‑term customer relationships, building trust and respect.
Privacy and Security – understand the importance of protecting patient information.
Vital Statistics – knowledge of birth certificate and paternity acknowledgement.
Organization – prioritize assignments, multi‑task efficiently, and execute work methodically.
Quality Orientation – consider all aspects of tasks, ensuring accuracy and thoroughness.
Work Independently – self‑supporting, requiring minimal supervision to complete tasks.
Education and Experience
Minimum: High School diploma or equivalent.
Preferred: Two (2) years’ experience in a hospital Health Information Management Department.
Certificates, Licenses, and Registrations
Preferred: RHIT or RHIA Certification.
Physical Demands
Ability to sit for extended periods.
Efficient use of computer keyboard and mouse.
Ability to lift up to twenty‑five (25) pounds.
Work Environment
Pleasant indoor environment.
Light physical effort.
Intense mental concentration and stress.
Exposure to infectious conditions and minor hazards such as muscle sprains, cuts, and bruises.
