Nurse Practitioner or Physician Assistant - Pediatric Cardiothoracic Surgery at Duke Health. The Duke Hospital Pediatric Cardiothoracic Surgery (CT surgery) Advanced Practice Provider (APP) provides and coordinates clinical practice for congenital cardiovascular patients in collaboration with the Pediatric Cardiovascular and CT Surgery teams. The APP delivers medical services to inpatient and outpatient settings, participates in surgical consultations, and may assist in the operating room or on Ex-OR cases as needed, while contributing to excellence in patient care, research, teaching, and leadership within the organization. The Duke Hospital Pediatric Cardiothoracic Surgery (CT surgery) Advanced Practice Provider (APP) provides/coordinates clinical practice for congenital cardiovascular patients. The APP provides medical services to patients both inpatient and outpatient by collecting and documenting patient data, conducting or ordering diagnostic and therapeutic procedures, providing direct patient care services, ordering and scheduling laboratory and imaging studies, participating in surgical consultations, assisting in the operating room or on Ex-OR cases when needed, and entering STS data. The APP contributes to excellence in patient care, research, teaching, and provides leadership to the organization. The APP will be utilized in the management of congenital heart disease patients at Duke University Health System in the operating room and the inpatient and outpatient setting. Responsibilities
Round daily on patients to assess, diagnose, plan treatment, set priorities and realistic outcomes, and evaluate the effectiveness and cost efficiency of patient care in collaboration with attending(s) and/or fellow/residents. Provide and coordinate clinical care for service-specific patients through established protocols under the supervising physician or backup physician who is on site or available by pager. Collaborate with residents, fellows, attending physicians, other APPs, nurses, and staff to formulate treatment plans and monitor patient progress. Perform procedures specific to the work environment after demonstrated competency and within scope of practice and credentialing protocols. Assess patients for changes in status and institute appropriate interventions; initiate emergency interventions as needed to stabilize patients; manage postoperative stabilization and care. Develop patient care pathways, protocols and guidelines and analyze clinical practice to improve care quality, effectiveness, and efficiency. Communicate relevant patient information to supervising MD, nursing staff, patient resource manager, home health agency, and other providers; document in the patient\'s medical record including diagnostic results and response to therapies/interventions. Utilize prescriptive privileges for identified patient populations; prescribe, order, and administer medications as appropriate; role-model competency in advanced practice skills. Provide daily handoffs to the appropriate attending, APP, fellow, resident, or intern at shift end. Educate patients, families, and staff using clear, non-technical language; document education in the medical record; conduct educational programs for DUHS patient care staff and other providers. Support clinical research and participate in interdisciplinary quality improvement and research activities; incorporate evidence-based decision making into practice. Participate in measuring and documenting outcomes and contribute to standards of care, guidelines, and policies within area of specialization; consult with and refer to other healthcare providers as appropriate. Qualifications
Education: NP: Graduate of accredited MSN or DNP Acute Care Pediatric Nurse Practitioner program; PA: Graduate of ARC accredited Master\'s or Doctoral degree in Physician Assistant Program. Licensure/Certification: NP: RN license with NP scope; PA: PA-C licensure; current unrestricted licenses as applicable by NC regulations. Experience: Prior experience as a Nurse Practitioner/Physician Assistant in a pediatric acute care clinical setting preferred, with cardiac background; new graduates welcome to apply. Prior OR/surgical experience required. Life support: BLS; PALS; ACLS recommended. Physical/Mental requirements: Ability to articulate scope of practice, work collaboratively, communicate clearly, and perform necessary assessments and documentation; ability to navigate hospital systems and provide clinical leadership.
