Co-Occurring Mental Health and Substance Use - IOP/PHP - Outpatient Therapist - Hybrid!
Square Medical Group (SMG) is a private behavioral health agency dedicated to providing compassionate, person‑centered care to adults with co‑occurring mental health and substance use disorders. We serve primarily MassHealth clients and operate over 1,000 outpatient care units per week.
Base Pay Range $62,000.00/yr – $70,000.00/yr
Program Overview SMG delivers medication management, individual, couple, and family therapy, virtual services, IOP/PHP, transcranial magnetic stimulation, in‑house lab services, and two levels of housing (Medicaid‑reimbursed Residences and self‑pay sober homes).
Role Summary We are seeking a compassionate, clinically skilled Outpatient Therapist to provide individual and group therapy to adult clients (18+) with mental health challenges, substance use disorders, or co‑occurring conditions. This hybrid role supports both in‑person and remote work with a focus on person‑centered care and measurable outcomes.
Key Responsibilities
Provide individual, couples, or family therapy to adult clients using evidence‑based approaches.
Conduct weekly stand‑alone group treatment for adult clients experiencing mental health or substance use disorders.
Lead IOP/PHP groups, including milieu therapy.
Perform comprehensive clinical assessments in compliance with SMG standards and payer requirements.
Maintain accurate and timely documentation of all encounters, treatment plans, and progress notes per SMG policies and Massachusetts regulations.
Support clients in achieving recovery goals by integrating community resources and coordinating care with medical and psychiatric providers.
Deliver a full caseload each week (up to approximately 25 individual therapy sessions or 12 IOP/PHP groups, with caseload adjustment based on group participation).
Engage in clinical supervision, team meetings, and case consultation for ongoing learning.
Respond to client needs by delivering outpatient, IOP/PHP, or residential group therapy as appropriate and maintain person‑centered care.
Uphold ethical, legal, and professional standards, including confidentiality and licensing compliance.
Specific Tasks
Conduct one‑on‑one or group counseling sessions as needed.
Complete intake assessments and collaborate on treatment planning for new clients.
Maintain accurate attendance records and centralized data tracking.
Coordinate with outside providers, case managers, and medical professionals for continuity of care.
Participate in daily huddles and regular clinical supervision.
Perform member outreach to reduce no‑show rates.
Assist with documentation workflows, quality improvement initiatives, and staff communications.
Take scheduled breaks to support self‑regulation and model healthy boundaries.
Qualifications
Masters Degree in Social Work, Mental Health Counseling, Art, or Expressive Therapy.
Experience using Electronic Medical Records (EMR); InSync preferred.
Previous experience with adults with co‑occurring mental health and substance use disorders.
Licensure preferred (LCSW, LICSW, LMHC).
Group therapy experience preferred.
No on‑call coverage required.
Schedule
Full‑time, 40 hours per week. Flexible scheduling.
Hybrid model: work from home or office locations in Greater Boston (Woburn, Weymouth, Springfield).
