As our Geriatrics Social Work Coordinator, you’ll have a key role in enhancing the well‑being and quality of life for our residents with compassionate care coordination and community‑building.
This position requires a bachelor’s degree in social work with direct and recent geriatrics experience OR five (5) years of experience as a Certified Geriatric Case Manager.
In this vital position, you will coordinate assessments, planning, and access to services, ensuring residents receive the support they need to thrive. Working closely with cross‑functional teams, you'll help residents meet Conditions of Occupancy while fostering a strong sense of community. Duties
Welcome and orient new residents, introducing community services and safety measures.
Assess resident needs and coordinate internal and external support services (e.g., home care, hospice, mental health).
Maintain and manage essential documents such as Physicians Order for Life-Sustaining Treatment (POLST), Durable Power of Attorney (DPOA) and Vial of Life.
Facilitate cross‑departmental collaboration to support resident well‑being and safety.
Guide residents and families through transitions, including end‑of‑life planning and ethical concerns.
Monitor care outcomes using Key Performance Indicators (KPI) and Objectives and Key Results (OKR) and prepare monthly reports.
Support transitions between Independent Living and Assisted Living, ensuring continuity of care.
Build relationships with hospitals, skilled nursing facilities, and community partners.
Present educational topics to residents and staff and contribute to outreach efforts.
Qualifications
Bachelor’s degree in social work from an accredited institution with direct and recent Geriatric experience OR a minimum of five years’ recent experience as a Certified Geriatric Case Manager.
Minimum three (3) years’ experience in serving older adults in a community‑based setting is required.
Understanding and demonstrated experience in working with older adults, dementia, aging issues, complex family dynamics and transition plans.
Familiarity with Assisted Living and interdisciplinary team approach, knowledge of community services and programs. (preferred)
Working knowledge of Window's based computer systems and Microsoft office products.
