Under the direction of a registered nurse or physician and under assigned administrative supervision, provides care within the scope of the Vocational Nurse Practice Act and regulatory standards. Assists in the assessment, planning, implementation and evaluation of patient care. Follows patient identification procedures, provides appropriate information for all patient procedures, and collects and handles specimens. Assesses and implements prescribed interventions for the patient. Implements and documents discharge plan for each patient. Recognizes that ethical/legal/medical dilemmas exist in the nursing practice and confers with the appropriate resource personnel when making ethical nursing decisions. Participates in maintaining a safe patient care environment. Assists with the care and maintenance of department equipment. Completes and accurate charting of patient records. Maintains accurate record keeping of equipment and quality measures. Collaborates with other disciplines of the health care team to ensure the delivery of care across the continuum, providing timely, safe and quality care. Responsible for providing a safe, HIPAA compliant environment for the patient and staff. Performs other duties as assigned. Must perform phlebotomy if applicable to department. Specialty Clinics Only
Performs specialty procedures that include catheters, injections, Mohs surgical patients, and injection preparation. Coordinates home meds, specialty pharmacy, DME and home care. Responsible for coordination of complex care. Qualifications
Education and Experience
High School diploma or equivalent. 2+ years’ experience as an LVN Basic keyboarding skills and knowledge of Microsoft Office Suite, including Outlook, Word and Excel Basic knowledge of medical terminology Experience with Electronic Medical Records (EPIC) preferred Primary Care and Urgent Care: Completion of educational courses before moving into an LVN II role (if hired into an LVN II, courses need to be completed within 6 months) Specialty Clinics: New grad with previous patient care experience (6+ months) will be considered in lieu of 2+ years of LVN experience License Required: Current LVN License to practice in the State of California Certifications Required: Current BLS Certification Certifications Preferred: Phlebotomy Certification About Us
