OTP Care Navigator (Case Manager) Overview
Evergreen Treatment Services (ETS) seeks an OTP Care Navigator to provide outreach, engagement, and case coordination for adults experiencing behavioral health challenges, homelessness, and substance use disorders. The role operates primarily from an external agency site (ORCA Center/DESC) alongside ETS’s Outreach team. Location & Schedule
Seattle Clinic and ORCA Center, Monday – Friday 6:00 AM – 2:30 PM Compensation
$34.85/hour ($72,500 annual) Key Responsibilities
Build trusting relationships with participants through respectful outreach and participant‑centered engagement. Provide direct outreach within partner agency locations, focusing on underserved high‑barrier populations. Coordinate care across systems, developing and maintaining strong relationships with partner agencies and community programs. Advocate for participant needs within service systems to reduce barriers and promote continuity of care. Support participants transitioning from ORCA Center care to Opioid Treatment Program (OTP) in alignment with recovery goals. Assist in completing the New Patient Orientation group. Monitor medication dispensing appointments, document encounters, and consult with treatment teams. Collaborative Team Duties
Communicate with medical providers regarding external agency communications. Register prospective participants and coordinate intake assessments at ETS. Act as telehealth equipment chaperone during remote medical provider intake. Coordinate care with partner OTPs, including participant transportation. Qualifications
Four years of college coursework in a relevant academic area and one year of appropriate experience. Bachelor’s degree in Psychology, Social Work, Sociology, or related field required. Experience working with individuals experiencing homelessness, substance use disorders, and behavioral health challenges. Valid driver’s license. Strong interpersonal skills and proficiency in motivational interviewing and harm reduction principles. Ability to navigate complex care systems and advocate effectively for participant needs. Excellent organizational skills, attention to detail, and ability to prioritize workload. Physical Conditions & Equipment
Employee may be exposed to illicit drug residues, fumes, or other bio‑hazardous materials and bloodborne pathogens. ETS provides training to limit risk. Regularly required to sit, stand, walk, and occasionally lift up to 30 pounds. Computer/laptop, photocopier, fax machine, phone; agency vehicle. Inclusivity & EEO Statement
