Case Manager / Counselor
The Champaign-Urbana Public Health District (CUPHD) is seeking a
Case Manager / Counselor
for a full-time (40-hour week) position in the Champaign office to provide HIV prevention and medical case management services. The selected candidate will perform professional work in securing, providing, coordinating, maintaining, and evaluating other supportive and referral services.
Prevention responsibilities include:
HIV and STI testing (including phlebotomy), counseling in-house and at outreach sites, including the CUPHD mobile program, bar outreach, jail, methadone clinic and other outreach sites as determined. Coordinate, evaluate, and maintain supportive and referral services Identify resources and referrals for intervention, prevention care management, health, and social services for clients Additional responsibilities include the development, implementation, and evaluation of HIV prevention assignments.
Medical case management responsibilities include:
Ensure clients with complex needs receive timely coordinated services which assist a client's ability to function independently. Assess the needs of the client, the client's family, and the client's support system; as well as arrange, coordinate, monitor, evaluate, and advocate for a package of multiple services to meet the specific client's complex needs. Evening and weekend hours will be expected Other assignments given as needed to support the Teen and Adult Services team.
Requirements:
Two (2) years of professional work-related experience Or graduation from an accredited college or university with a bachelor's degree in human services, or a closely related field Persons with lived experience will be prioritized. Valid driver's license, dependable transportation, and proof of automobile insurance. Spanish speaking are highly encouraged to apply

The expected annual starting salary is $52,556. Full annual salary range for a Class 6 Case Manager is $52,556 - $70,950 for a 40-hour week.
