Intake Coordinator SCHEDULE:
Full Time JOB POSTING: Provides direct services for agency clients to include but not limited screening for admission, comprehensive clinical assessments, person-centered planning/treatment planning, crisis planning, crisis intervention, individual and group therapy, and case management. Provides intake coordination to include review of intake paperwork, schedule coordination, and training. Manages open access/walk-in services. Manage coordination of discharges for new and established individuals in order to ensure a smooth transition of care. REQUIRED
EDUCATIONAL/EXPERIENTIAL
QUALIFICATIONS: Graduation from an accredited University with a master’s degree in human services and two (2) years of progressive experience in behavioral health assessment and treatment. Full or Associate licensure as a Psychological Associate, Professional Counselor, or Clinical Social Worker. Dual licensure as a Clinical Addictions Specialist preferred. POSITION HIGHTLIGHTS: Perform intake, assessment, referral, and evaluation of the social and daily living needs of clients of the outpatient treatment program. Perform triage and screening protocols for clients to ensure the appropriate next steps into treatment. Reviews documentation created by the intake counselors. Coordinates schedules of clinicians performing intakes. Provides clinical supervision and feedback to intake counselor as appropriate. Provides administrative supervision to intake staff Provide training for all new intake counselors. Provide direct services through community agencies and contacts for clients and coordinates and works closely with the treatment staff in providing services for clients from various communities for substance abuse and mental health treatment. Provides supportive and informational counseling to include crisis intervention, case management, and the development of collateral contacts. Establishes network of agency contracts and resources in order to fulfill the socioeconomic needs of the clients. Provides independent diagnostic, psychosocial and substance abuse evaluations and counseling which focus on complicated substance abuse and addiction as well as the issues generated from its impact on daily living and individual and family relationships. Provide in-depth counseling, which can range from crisis intervention to assessment, referral, and group / individual Coordinate new referrals into services. Maintain a working relationship with community partners in order to better coordinator new
referrals/discharges
for services. Provide coordination of discharge planning for new and active clients to ensure a smooth transition of care to the appropriate level of care. The caseload is characterized as complex with a preponderance of methadone clients and clients with co-occurring disorders. Provide information to and confer with criminal justice system, employers, and families on treatment plans, behavior management, and other problems and needs. The counseling is best characterized as individual and group utilizing a variety of treatment modalities depending on the needs of the client. Subject to inside environmental conditions and exposed to fumes, odors, dusts, mists, gases or poor ventilation and subject to work in close quarters. Employee may be exposed to human body fluids and is subject to the OSHA requirements on blood borne pathogens. ADDITIONAL INFORMATION: Thorough knowledge of substance abuse and mental health assessment techniques, DSM-V diagnostic classifications, and person-centered planning. Considerable knowledge of drugs and their effects and related disease and health issues including addiction and the impact in the community. Considerable knowledge of available community resources, services and the coordinating efforts needed for clients with substance abuse problems. Ability to determine appropriate community services and agencies needed to meet client needs. Ability to network and coordinate efforts between outreach, treatment, and other support systems for the clients. Considerable knowledge of the practical application of different therapeutic approaches, psychological interventions, group dynamics, crisis intervention, and family therapy. Knowledge of the psychosocial, socioeconomic and behavioral problems and their treatments. Ability to formulate treatment goals and to record concisely required documentation. Knowledge of laws, regulations, and policies that govern the program. Knowledge of medical terminology, disease processes and psycho-pharmacology. Skill in establishing rapport with a client and/or family and applying techniques of assessing psychosocial, behavioral, and psychological aspects of the client. Knowledge of various methods of psychological evaluation, including, but not limited to, conducting semi-structured diagnostic clinical interviews. Ability to communicate effectively in oral and written forms. Must be able to physically perform the basic life operational functions of climbing, stooping, crouching, reaching, pushing, pulling, lifting, standing, walking, fingering, grasping, talking, and hearing. Must be able to perform sedentary work exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift objects. Must possess the visual acuity to perform extensive reading and determine the accuracy of work. Must be proficient with computer systems.
