Gateways Hospital and Mental Health Centers' Enhanced Care Management (ECM) program is a newly certified program serving Managed Care Plan members with chronic mental health and social support needs. Candidates will have the opportunity to be a part of an exciting startup phase which will include new program operations infrastructure development and implementation, recruiting, hiring and training program staff, interface with managed care plan managers, supporting in‑community client outreach and engagement, and establishing collaborative partnerships with community stakeholders to enhance closed loop referrals and whole‑person client services.

SUMMARY OF POSITION The ECM Program Director will work closely with organization leadership to help lead and develop and manage the Enhanced Care Management (ECM) program. The director is responsible for managing the daily operations of the department, providing leadership and guidance to the staff, and ensuring that all policies and procedures are followed. They will work closely with other internal and external stakeholders to provide seamless care to clients and ensure that all services are delivered in an efficient and effective manner.

Works in collaboration with: Care coordinators, care navigators, case managers, medical/health care providers (primary care physicians, nursing staff, mental health workers), community support organizations

ESSENTIAL DUTIES

Reporting to the Director of Business Development, responsible for day‑to‑day operations of the ECM program, including budget oversight, training and workflow development and recruitment

Oversight and guidance for direct reports and oversight of all downstream and front‑line staff in the department

Responsible for maintenance of quality and risk mitigation standards and plan for accounts in coordination with leadership, including assessing the effectiveness of strategies deployed

Oversight of all required compliance reporting and auditing activities for the ECM program

Directly hire or guide staff to hire, develop and lead a high performing team of clinical and non‑clinical professionals that will deliver exceptional support for providers and care for the patient population while meeting quality, financial and client enrollment targets

Develop and implement operational population health methodologies, including development of process- and outcomes‑related key performance indicators (KPIs) for program operations and outcomes

Provide effective leadership to stimulate a high performing culture

As an operational leader, embrace and contribute to Gateways culture of process improvement with a focus on streamlining processes to add value to the organization

Develop internal strategic initiatives, with aligned KPI to drive program success in managing related cost, quality, daily operations, and outcomes of member care coordination

Develop process to engage new and retain current clients to ensure the ECM program is meeting enrollment targets monthly

Ensure compliance with all local, state and federal regulations and ethical standards

Develop and implement policies and procedures to ensure quality care and compliance with regulatory guidelines

Provide leadership and guidance to ECM staff, ensuring they are meeting performance standards and goals

Oversee the coordination of care for clients, including assessments, care plans, and follow‑up care

Ensure that all documentation is completed accurately and in a timely manner

Monitor and evaluate the effectiveness of the ECM program and services provided

Analyze data and provide reports to leadership on the performance of the programs as well as required reporting to managed care plans

Proven success managing budget including labor, expenses & capital expenditures

Flexibility to work weekends & evenings as the business requires

EDUCATION & CERTIFICATES

Master’s degree in an applicable field (e.g., MBA, MPH, MHA) strongly preferred or equivalent work experience in a healthcare and/or behavioral healthcare management position

EXPERIENCE/QUALIFICATIONS

5 – 10 years of experience in a health care setting, preferably focused on working with high needs and complex care clients

Experience with start‑up or new program development highly preferred

Strong communication and interpersonal skills; experience in managing stakeholder relationships

Experience and desire to train, motivate, develop, and lead diverse staff

High level of proficiency in Excel and other Microsoft Office programs

Skills related to effective project management, including strong work ethic, attention to detail, time management, ability to prioritize, problem solving, flexibility and willingness to learn

Experience with and deep understanding of all aspects of the California CalAIM program including ECM and Community Support (CS) service delivery

Prior experience in at least two of the following areas strongly preferred:

Clinical Redesign

Behavioral Health

Population health management

Regulatory compliance (Medicare, Special Needs Plans, Medicaid, etc.)

Care model redesign for providers and payers

Care Management / Medical Management Care Models

SDOH and Community‑based organization (CBO) integration

REQUIREMENTS

Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI), and Community Care Licensing (CCL) background clearance

Valid California Driver’s license

TB clearance

Driving record acceptable for coverage by Gateways insurance carrier
