Screen/evaluate patients within short-term and long-term rehab and outpatient opportunities Utilize standardized assessments and evidence-based practice to support clinical interventions Provide rehabilitative, skilled, and medically necessary treatment intervention to patients in short and long-term settings Administer a wide variety of interventions as part of the plan of care based on individual patient needs Assess patient progress and adjust treatments in the clinical record per regulatory and clinical practice requirements Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders Communicate with the OneClinical Team regarding patient progress and barriers Provide comprehensive discharge summaries per regulatory and clinical requirements Qualifications
