Enhanced Care Management Program Manager - Northern California Region
Base pay range: $80,000.00/yr - $90,000.00/yr Location: Northern/Central California (regional leadership role for ECM). Overview
At Pacific Health Group, the ECM Program Manager leads the Enhanced Care Management program, guiding regional teams to provide compassionate, person-centered care that improves member outcomes and advances health equity. Responsibilities
Team Leadership & Supervision
Directly supervise and mentor Lead Care Managers across multiple counties in California (Northern/Central regions). Conduct weekly 1:1 meetings with staff for guidance and goal alignment. Facilitate weekly team meetings to promote collaboration and communication. Meet in person with Lead Care Managers regularly to maintain leadership presence and program alignment. Conduct performance evaluations and provide feedback to optimize team performance.
Program Development & Quality Assurance
Establish benchmarks and performance metrics for the ECM program. Design and implement QA protocols to monitor service delivery and outcomes. Develop standardized workflows across counties and monitor improvements with monthly reporting.
Member Services Oversight
Ensure Lead Care Managers conduct regular in-person visits with ECM members and monitor interaction quality. Improve member engagement and satisfaction through targeted strategies.
Cross-Functional Collaboration
Serve as liaison between ECM and hospitals, primary care, and community partners. Collaborate with data analytics to track outcomes and identify improvements.
Budget & Resource Management
Develop and manage the ECM program budget and resource allocations across counties. Track expenditures and report on ROI.
Program Innovation & Growth
Identify opportunities to enhance program effectiveness and scalability. Develop proposals for program expansion or new services; stay current with best practices.
Additional Focus Areas
Lead weekly meetings structure, one-on-one meeting practices, mentorship, and cross-county collaboration to strengthen program delivery.
Qualifications
Education & Experience
Master's or Bachelor's degree in Healthcare Administration, Public Health, Nursing, or related field. 5+ years of healthcare management experience, with at least 3 years in care/case management services. Minimum 2 years of direct experience in Enhanced Care Management programs or equivalent. Understanding of ECM program requirements, populations served, and best practices for high-need Medicaid populations. Supervisory experience managing teams across multiple locations. Knowledge & Skills
Strong understanding of care management principles and healthcare delivery systems. Familiarity with California ECM program structure, reporting, and quality metrics. Experience interpreting ECM policy guidance from the Department of Health Care Services. Analytical skills for evaluating program effectiveness and driving improvements. Proficiency in healthcare data analysis and quality improvement methodologies. Personal Attributes
Strong leadership, communication, and interpersonal skills. Ability to manage multiple priorities in a fast-paced environment. Commitment to person-centered care and health equity. Working Conditions
Regular travel (50-60%) to county sites throughout California. Occasional evening/weekend availability for special initiatives or urgent needs. Benefits
