Northwestern Memorial Hospital . The Physical Therapist role includes direct patient care, teaching, participation on appropriate committees, and departmental process improvement, reflecting the mission, vision, and values of NM and complying with applicable policies, procedures, guidelines and regulatory standards. Responsibilities

Demonstrates the four NM values of teamwork, patients first, integrity, and excellence in daily work and interactions. Presents a friendly, approachable, professional demeanor and appearance. Provides accurate information and timely updates to patients and customers; addresses questions and concerns promptly; directs patients and customers to appropriate resources. Uses effective service recovery skills to solve problems or service breakdowns when they occur. Maintains primary responsibility over patient plan of care through timely and consistent collaboration with treating PT/PTA(s). Demonstrates teamwork by helping co-workers within and across departments; communicates effectively, respects diverse opinions and styles, and acknowledges contributions. Uses organizational and department resources efficiently; manages work schedule to complete tasks on time. Contributes to continuous improvement efforts; participates in efforts to reduce costs and streamline processes; supports department goals. Demonstrates problem solving and adaptability when treating patients with special circumstances, supported by team feedback and documentation. Maintains essential skill competencies in evaluating and selecting appropriate treatment modalities, equipment, and protocols; performs re-evaluations per department guidelines; documentation supports reimbursement. Assumes responsibility over the course of PT patient management from initial contact to discharge, maintaining collaboration with treating PTA(s). Adheres to department policy for evaluations, progress notes, and discharge summaries; notes are documented in EMR; completes daily notes reflecting patient response and status relative to the treatment plan. Provides educational materials appropriate to patient understanding; participates in committees, department processes, and special projects as per performance plan. Develops professional relationships with healthcare team members and participates in internal/external marketing as required. Maintains a culture of respectful communication; accepts constructive feedback and makes timely changes as evidenced by coaching forms/chart audits. Completes non-patient care responsibilities within established timelines (coverage notes, schedule changes, etc.). Contributes to department educational efforts (e.g., in-services, journal clubs, orientation, PI) as per expectations. May participate as a clinical instructor, mentor, or observation host for student programs; maintains productivity standards. Demonstrates flexibility in schedule or assignments to meet patient care needs and department meetings outside normal hours. Qualifications

Required

Bachelor’s Degree in Physical Therapy. Licensed Physical Therapist in State of Illinois. BLS from American Heart Association upon hire. Preferred

Masters or Doctoral Degree in Physical Therapy. Equal Opportunity
