About the Role
Provide psychiatric evaluations and medication management to patients in skilled nursing and long-term care facilities
Collaborate with an interdisciplinary care team to enhance patient outcomes
Develop and implement individualized treatment plans tailored to each patient's needs
Build lasting relationships that improve both patient well-being and facility outcomes
Qualifications
Active and unrestricted California nurse practitioner license (PMHNP certification required)
Active and unrestricted DEA registration
Minimum 6 months of psychiatric medication management experience
Excellent clinical assessment, communication, and collaboration skills
Genuine passion for geriatric and post-acute mental health care
